| Purposes:Surveyed the mental state, psychological problems, reasons and the influences arising on the thyroid cancer patients during the surgery, in order to explore the effect of cognitive intervention measures in thyroid cancer patients on their psychological state. Applied the collective interventions and individual interventions to thyroid cancer patients education,and helped patients rebuild their cognitive model, improve their negative state. And provided a scientific theoretical basis for psychological intervention in clinical nursing care. Methods and contents:In this study, according to the inclusion and exclusion criteria for data collection, depending on the Random Number Table, thyroid cancer patients were divided into the experimental group and control group. Patients were from the First Hospital of Jilin University Thyroid Surgery. The control group was given the conventional admission, discharge and post-operative instructions missionary only, while the experimental group received the cognitive interventions, including collective and individual interventions. Through Semi-structured interview to explored the patients′ psychological and the reasons. And introduced the thyroid(cancer) related knowledge, ward environment, the surgical and other relevant circumstances. In order to ease the negative psychological. Individual interventions are carried out by conversation once or twice a day, and each time is about 15 to 20 minutes. Collective interventions carried out by playing PPT and video, 5 or 6 days after the surgery. Used the General Information Questionnaires to explore the general situation, patients filled out the SCL-90 respectively when they were admitted and left hospital. Used Excel and SPSS17.0 to compare the scores of the two groups, including the constituent,t test,χ2 test and variance analysis test. P<0.05 is statistically significant. Result:There were 200 cases of eligible patients enrolled in this investigation, they were divided into two groups, depending on the Random Number Table, there were 102 invalids in the experimental group includes, and 98 invalids in the control group. Patients were from the First Hospital of Jilin University Thyroid Surgery. The results were showed in the followings:1. On the day of admission, statistically, there was no difference(P>0.05)between the experimental and the control group, in the following aspects such as SCL-90 scores, gender, age, occupation, education level, marital status,income, medical expenses payment, smoking, drinking and thyroid disease, it was comparable.2. comparing with the Norms(N=1388),the patient somatization, interpersonal relationship, anxiety and depression scores were higher than the Norms, there was statistically significant difference(P<0.001).The obsessive-compulsive symptoms, hostility, terror, paranoia, psychotic scores lower than the Norms.3. The age, education level,income and payment were the important influences to patients′negative state.4. After interventions, there was comparability(P<0.001)on the part of the somatization, anxiety, depression, interpersonal sensitivity, obsessive-compulsive symptoms, hostility and terror scores. Paranoia and psychosis have no statistic difference(P > 0.05). Probably they were related to inclusions and exclusion criteria, even the duration of the interventions.5. The control group patients had a certain improved,but not as good as the experimental group. Conclusion:1. Thyroid cancer patients has some psychological problems.2. Thyroid cancer patients′ negative state related to the age, education level,income and payment.3. Cognitive intervention can improve thyroid cancer patients′ negative psychology obviously.4. Cognitive intervention can help thyroid cancer patients reconstruction of a new cognitive model, reduce negative psychology, and can be applied to the clinical. |
