Retrospective Study Of Clinical Diagnosis And Treatment Of Synovitis Of Wrist

Objective: To study the diagnosis and treatment of synovitis of wrist joint and and treatment effect analysis.Methods: Surgical treatments were performed in 65 cases with synovitis of wrist joint from November 2010 to may 2014.Their clinical history including diagnosis,pathological classification and treatment,were investigated.Results: The group of 65 cases, 39 cases were male, 26 female; age 10-78, with an average of 34 years. The onset location: left in 29 cases, 27 cases of right, both wrist in 9 cases;17 cases of patients with injury history, onset to treatment time 0.5months-14 years, average 33.9 months; wrist active 0-115°, average 51.8°. All patients underwent synovectomy, postoperative pathological diagnosis: chronic synovitis in29 cases(44.6%), 18 cases of rheumatoid synovitis(27.7%), 9 cases of tuberculous synovitis(13.8%), 4 cases of gout synovitis(6.2%), bacterial synovitis in 2 cases(3.1%), pigmentation villonodular synovitis in 1 case(1.5%), 1 case of fungal synovitis(1.5%) and tuberculosis combined with bacterial infectious synovitis in 1case(1.5%). According to the Mayo Wrist Scoring Chart, the group was outstanding in 12 cases(27.3%), good in 16 cases(36.4%),acceptable in 13 cases(29.5), poor in 3cases(6.8%), the overall excellent rate was 63.6%. The VAS score shows that the patient’s postoperative effect is outstanding 75%, good 22.7%, acceptable 2.3%,patients with pain improvement rate was 97.7%.Conclusion:1.In the diagnosis of synovitis of wrist joint,simple chronic synovitis is most common,followed by rheumatoid synovitis and tuberculous synovitis.2.Drug therapy is the main treatment method for Synovitis of wrist joint, if drug therapy is invalid or bone destruction happend, early surgical treatment is needed.3.Wrist joint synovial resection has obvious effect in the change of clinical symptoms, preventing bone destruction, protecting the wrist function.
