A Study On Imaging Features And Treatments Of Temporomandibular Joint Synovitis

Objective: To investigate the panoramic radiograph imaging features of bilateral joint space with people diagnosed as unilateral temporomandibular joint synovitis(TMJS)and compare three different therapeutic approaches,clinical effects on this disease.Methods:Twenty-seven patients diagnosed with unilateral TMJS collected from the First Affiliated Hospital of Xinjiang Medical University were accepted with panoramic radiograph.The sagittal oblique panoramic radiograph on bilateral TMJ space was recorded by digital surface tomography X-ray machine in mouth intercuspal positon and reconstructed by CliniView 9.2 software,bilateral TMJ space indexes were measured to compare the bilateral joint space differences.Fifty-six TMJS patients were collected subsequently and divided into three therapeautic groups randomly,group 1 named supra-articular cavity sodium hyaluronate(HA),injection,group 2 named HA,s injection combined local Votalin emulsion application,group 3 called HA,s injection combined stabling splint.VAS score and temporomandibular joint dysfunction were detected before and after a course treatment to compare three therapeautic treatment effects.Results: The anterior,superior and posterior condylar space in the lesion side were significantly widened than those in the unaffected side(P <0.05),which were increased by about 32.46%,64.62%,55.05%respectively.The superior space showed a great significance on the asymmetry of the condylar space.An obvious decreased VAS score revealed in group 2 and group 3compared with group 1(P<0.05).As for the Fricton index,there was also a difference for the DI value and CMI value among the three groups(P <0.05),however,the PI in group 3decreased obviously(P<0.05).Conclusion: The condylar space of patients with unilateral TMJS in the lesion side is wider than that of the unaffected side,The superior space has a greater influence on the symmetry of bilateral joint space.All the three methods havetherapeutical effect on TMJS,while HA,s articular injection combined with stabling splint is more effective on the TMJS by relaxing the masticatory muscles.
