Compliance And Influencing Factors Of Defecation Function Training For Patients With Ultra-low Rectal Cancer After Intersphincteric Resection

ObjectiveTo analyze the compliance of defecation function training of patients with ultra-low rectal cancer after intersphincteric resection and the influencing factors.And therefore to provide theoretical reference for the problems and nursing management of training. MethodsThe study was a self-control study.Patients with ultra-low rectal cancer after intersphincteric resection were treatment with the questionnaire from January 2014 to the December 2015 in one tertiary hospital in Changchun city Jilin Provence.The enterostomal therapist educate patients with defecation function training.Research use the basic information questionnaire, compliance evaluation of defecation function training,social support rating scale,defecation diary and the fecal incontinence quality of life questionnaire as research instrument.The date was input by Excel and analyze by SPSS 17.0.Using the descriptive analysis, chi-square test, Spearman correlation analysis,ANOVA and multiple linear regression to analyze the influencing factors of defecation function training for patients. Results1.The score of the defecation function training compliance was(67.67 ± 5.28);only 21.25% patients’ compliance of defecation function training were high,56.25% patients’ compliance were medium.Among the average scores,the score of disease knowledge was lower,the score of rehabilitation training was the lowest.2.Influencing factors of defecation function training were age,degree of education,each person income of family per mouth,social support,pathologic stage,operation method,intersphincteric resection,com-plication,pain(P<0.05 or P<0.01).3.Anal function and the fecal incontinence quality of life was in positive correlation with the compliance;At 6 and 9 months,anal function of patients who has good compliance was obviously improved(P<0.05).4.Multiple linear regression analysis showed that degree of education,use of support,pathologic stage,intersphincteric resection comp-lication,pain were the major causes. Conclusions1.The compliance of disease knowledge rehabilitation training were poor.It suggested that nurses should give them effective health education at the time of hospitalization and sustainable guidance out of hospital.2.The compliance of defecation function training of patients are related to anal function and the fecal incontinence quality of life.A effective training program and follow-up appointment is very important.3.Degree of education,use of support,pathologic stage,intersphincte-ric resection,complication,pain were the major causes of the compliance of defecation function training;Nurse should fully recognize those to improve the compliance and defecate function.
