Objective:According to the basic principle of cold therapy, compare and discuss the routine treatment and nursing after thyroid nodule microwave ablation on the basis of three kinds of temperature-4~0℃,1~4℃,5~8℃ ice bag for patients with wound local skin temperature, pain, anxiety,comfort and satisfaction of comprehensive influence, aims to seek ideal for patients with thyroid nodules after microwave ablation of ice temperature and ice bag suitable for changing time in order to achieve the best effect of cold, maximum reduce the patients’ postoperative pain, anxiety level, improve comfort and satisfaction, for science after microwave ablation in patients with thyroid nodule best cold solution provide preliminary trial basis and data support, in order to better guide clinical nursing work.Methods:This research use the convenience sampling method,90 patients with thyroid nodules of microwave ablation were selected in September 2015 to March 2016 at level 3 first-class hospital minimally invasive surgery in Harbin city as the research object, according to the inclusion criteria randomly divided into control group, experimental group A and group B, all three groups on the basis of routine treatment and nursing in hospital in strict accordance with their respective corresponding cold, cold temperature of the control group for 5~8 ℃, the experimental group A is 1~4 ℃, the experimental group B is-4~0℃, duration of cold all 8 h,the control group ice temperature up to room temperature after the replacement, the experimental group A, experimental group B ice temperature of more than 10~15 ℃ Cwhen replaced. In hospitalized patients received routine medical examination after signing the informed consent, based on skin temperature measurement, state anxiety inventory and comfort evaluation and in postoperative cold compress before 30 min measurement SF-MPQ pain score; on the basis of routine postoperative treatment and nursing implementation of cold, observation and records of patients with postoperative at different time points of local cold compress the skin temperature, SF-MPQ pain score, second days after the end of treatment again state anxiety inventory, comfortable degree and satisfaction degree evaluation. All the data for this study after double check entry SPSS 17.0 statistical software for statistical analysis. The inspection level a=0.05, P<0.05 difference was statistically significant. The effects of different cold temperature on patients with comprehensive analysis.Results:1. Before the intervention, the three groups of patients with general information, skin temperature, SF-MPQ pain score, S-AI and GCQ scores were compared, the difference was not statistically significant (P>0.05).2. After the intervention, the SF-MPQ pain score, S-AI and GCQ scores of the three groups were compared, and the differences were statistically significant (P<0.05);3. After the intervention, the skin temperature of this three groups decreased gradually with the time extension,then rising, until replace a new cold compress bag into the next cycle; After operation2h,4h,8h with the SF-MPQ score in three grops compared with statistically significant differences(P<0.05); S-AI score comparison among the three groups have statistical significance (P<0.05), multiple results showed that the control group and the experimental group A, the control group and the experimental group B difference was statistically significant; GCQ score comparison among the three groups only physiological dimension score difference has statistical significance (P<0.05), other aspects of the differences were not statistically significant(P>0.05); Multiple comparison results show that, experimental group B and control group, experimental group B and experimental group A difference has statistical significance(P<0.05); satisfaction compared display, the difference among the three groups have statistical significance (P<0.05), multiple comparison, control group and the experimental group A, control group and experimental group B had significant difference (P<0.05).Conclusions:1. Compare to the other two groups,-4~0℃ can significantly reduce the post operative pain degree and significantly improve patient comfort.2.-4~0℃ cold therapy group with 1~4℃ cold therapy group in reducing the anxiety level of patients and improve satisfaction has the same effects,3. Based on the above conclusions,-4~0℃ is suitable for the temperature of cold therapy, on average, about 50 minutes to replace a bag is more suitable, both prolong the life time of the ice pack, reduce the workload of nurses, and can significantly reduce the patients pain and improve the degree of comfort and satisfaction.
