Comparison Study Of Two Kinds Of Suture Methods Which Used In The ORIF Operation Of Calcaneal Fracture

Objective:Comparing Allogower-Donati mattress suture and vertical mattress suture in calcaneal fracture ORIF operation on suture method and complications of operation,to provide some basis for the suture mode selection in the calcaneal fractures.Methods:The calcaneal fractures cases were collected from People’s Hospital of Sichuan Provincial during May 2014 to February 2015,35 of them met inclusion criteria, and were divided into two groups randomly (the treatment group using Allogower-Donati mattress suture,the control group using vertical mattress suture), operated by same doctor with the standard ORIF operation, and same follow-up treatment. Comparing two groups of patients in the suture time, wound healing time, complications of grade, complication rate, postoperative scar the size of the area and wound healing scores on the first day, first week, second week, sixth week after operation, etc. The results were statistically analyzed by spss19.0.Results:The treatment group and the control group illustrated no statistical significance in the operation time, drainage time and wound healing time after analysis. In suture time, the treatment group was 4.45+0.26min, the control group is 5.02+0.39min, p=0.00<0.05, with statistical significance. Wound healing score of the treatment group and the control group on the first day after operation, first 1 week, second week and sixth week after operation, the calculated P values are greater than 0.05, there were no statistical significance.2 cases occurred complications in the treatment group, the incidence rate was 11.1%,3 cases occurred complications in the control group, the incidence rate was 17.6%, the incidence rate in treatment group was higher than the control group, with significant difference. But tested by the Fishers Exact Test (n<40, Texact<5), bilateral P approximate value=0.11>0.05, no significant difference between two groups. The scar size of treatment group was 0.87+0.13cm, while the control group was 1.48+0.13cm. The control group was bigger than the treatment group, p=0.00<0.05, with statistical significance. According to the distinction between the level of incision complications in complication group and cure group, the therapeutic effect was evaluated by rank sum test, results of which was z=-5.440.01, p=0.58>0.05, with no significant difference.Conclusion:In this experiment, there was no significant difference in the healing time and the the wound scores between the treatment group and the control group, but in terms of the suture time and postoperative scar size, the treatment group was significantly better than the control group. The level of incision complications and the clinical curative effect illuastrated no significant difference between two groups.The complication rate is significantly lower in treatment group than the control group.
