Objective:There are many factors affecting scar formation, such as tension, race, age, heredity, environmental factors, immune response and other factors play a certain role in its formation and development, among which tension factors are closely related to the degree of scar.In the first stage of wound repair,it is especially important to reduce the tension sufficiently. In recent years, many improved suture techniques have been proposed to improve the defects of traditional suture and provide better conditions for wound healing and reducing postoperative scarring. Methods:In this study,we proposed the method of in-situ recuperation vertical mattress suture, and compared it with traditional continuous intradermal suture through clinical experiment,it was confirmed that in-situ recuperation vertical mattress suture has better tension reduction effect than continuous intradermal suture. The widely used Patient and Observer Scar Assessment Scale(POSAS)was used for postoperative scar, and scar width was used as a separate measurement standard for statistical analysis. The scar scar of the incision 6 months after surgery was compared and analyzed. Results:Compared with the intragermal continuous suture, the in-situ recirculating vertical pad suture can effectively reduce the scar of the postoperative incision. The surgical incision healed well in both the experimental group and the control group. After 6 months of postoperative follow-up,the early depression of the recirculating point disappeared after3-6 months, with good recovery and no local pigmentation. The mean width of scar in the experimental group was smaller than that in the control group at 3 and 6 months after surgery. The design and t-test of the two groups showed statistically significant differences(P<0.05).The mean value of observer scores and patient scores in the experimental group were lower than those in the control group 3 and 6 months after surgery, and the difference was statistically significant between the two groups by design and t-test(P<0.05).Conclusion:In situ return stitch vertical mattress suture can provide a long-term tension reduction effect, although at the early stage of the wound edge valved and raised, return stitch location is local depression, the surface is not smooth, but with the passage of time,it will become smooth, and minimize the scar after healing. At the same time, this suture can also have a positive effect due to the decreased tension caused by the hydrolysis of absorbable sutures and the possibility of scar hyperplasia caused by the acute inflammatory response of tissues.
