How to overcome the negative influence to restrain the rapid growth of medical costs so that the insured patients can undertake the economy burden and to ensure the smoothly run of the medical insurance fund became a hot topic. The basic medical insurance payment, as the most advantageous weapon, has received considerable attention. We can see the reformation of the payment is approaching to a more scientific and reasonable way with the payment change its’single style to combined. Only one thing account for this occurrence is that an scientific payment not only can restrain the excessive growth of medical costs、ensure the quality of medical services、adjust the behavior of doctors, can also be a reasonable allocation of medical resources to guarantee social fairness.This article starts with the urban workers’ basic medical insurance and it’s payment. On the basic of the brief overview of the social basic medical insurance system and the payment, this paper gets further insight into four countreis’ payment, including American, Britain, Germany and Singapore, in order to find some implications. At last, an example, city of Wuxi is given to study it’s payment. By means of studying the payment we find that some achievement has been got, especially the increasing of the number of the insured and the fund. What’s more, it also decreased the burden of the patients. But also, some problems occurred as we expected. First of all, the formulate of the maximum amount of the prepaying system without considering the introducing of new technology. Secondly, some medical institutions prevaricate sever patients in order to keep profits. And thirdly, it seems a little slower to push forward implementing of single disease. With the conclusion, we draw up some measurements, including formulating an scientific maximum amount of the prepaying system、consummating all kinds of supervisory measures、 exploration of implementing composite payment and the establishment of insurance risk fund. All what we done is to elaborate the function of the payment and hope to do contribution to the innovation of the payment.
