Objective: To understanding the human resource status of community health service centers comprehensively in Urumqi. To analysis the intention and influence factors of medical personnel and residents for general practitioner signing service, in order to improve GPSS mode and provide reference for other community health service institutions. Method: Adopt the method of literature investigation, expert consultation and questionnaire investigation, in 59 CHSCs 1945 medicine personnel for base-line investigation, and using cluster sampling selected 5 CHSCs about 303 medicine personnel and 3762 residents by using questionnaire in Urumqi. Results: 1) For base-line investigation, we have been surveyed a total of 1945 medical personnel. The medical and nurse ratio is 1∶1.1. Age structure: doctors are mainly between the age of 30 to 45 years old(47.6%), nurses are mainly between the age of 18 to 30 years old(75.4%). Education structure: the doctor are mainly in college or above bachelor degree, the percentage were31.5% and 68.8% respectively. The proportion of nurses are mainly in junior college and secondary specialized school, the proportion is 40.6% and 64.5% respectively.Professional title structure: the doctors are mainly in middle and senior professional titles,the proportion is 43.1% and 63.9%, respectively; 2) In 303 medical personnel, 88.0%(269/303) was known the GPSS; 64.4%(195/303) of the medical personnel are willing to sign the service. Multiple-factor analysis found that whether known the GPSS, whether involved in health management skills training, whether have teaching experiences,whether can save manpower, improve residents medical treatment compliance and whether can close the doctor-patient relationship are affecting CHSC medical personnel to signing the service. The related factor ranks which influence medical personnel to signing GPSS is that, people didn’t have policy support(44.2%), and have relevant knowledge(33.3%) and skills for general medicine, and health management related knowledge and skills(9.2%); 3) In 3762 community residents, 46.0%(1711/3716) residents known GPSS;80.6%(2990/3710) are willing to sign general practitioner signing service. The main influence factors for residents to signing GPSS were: the doctor can be signing only in their own community(31.2%), the benefits for known the GPSS is low(25.9%), the credibility for the GPSS is low(21.0%). Multiple-factor analysis found that ethnic,occupation, whether have health care, whether suffering from chronic diseases, whether known what is the GPSS, whether willing to have the fixed GP, whether accept the GP as the core of first option, and whether have the superior medical center referral experience.Conclusion: For the human resources base-line investigation in Urumqi 59 GHSCs, age strutrue was in a reasonable distribution, but the distribution of ethnic and post structure is unreasonable, and education, professional title, business skills needs to be improved.The medical personnel and residents have a certain intention to sign the GPSS, policy support,skills training and the allocation of resources is the main factors which influencing the medical personnel, the medical institutions limited, the benefit of GPSS is low, and have little trust for GP are the main factors which influencing the community residents. we should optimize the allocation of resources, form a complete set of incentive mechanism;Strengthen the publicity of education, and establish the hospital- community linkage service, in order to meet the demand of community residents, and promote integration of training between general medical knowledge and health management skills. Create a suitable team which have Xin Jiang regional features, innovative and effective, in order to provide a solid foundation for GPSS.
