| Objective To discuss the therapeutic effect of triple pelvic osteotomy for the avascular necrosis of femoral head in childrenMethod Fifty-five patients with the avascular necrosis of femoral head in children in our hospital were treated by triple pelvic osteotomy. The series consisted of 48 male (50 cases) and 9 female (10 cases). Therefore,31 patients were right cases,23 patients were left cases,3 patients were bothsides cases. The mean age of the patients was 9 years and 2 month at operation. According to Herring’s classification,42 cases were graded as group B, and 18 cases were graded as group C.Results Totally 57 patients were followed up successfully for a period from 3 to 6 years, and were assessed by Mckay Criteria and Willett assessment. The clinical result presents that 40 cases are excellent (66.7%),13 cases are good (21.6%) and 7 cases are acceptable (11.3%), totally 88.3% of the cases is excellent or good. From the radiological results,it shows that the epiphyseal height, the fraction of coverage, center edge angle increased obviously(P<0.01), Sharp angle decreased(P<0.01), the Shenton’s line changes smoothly, and the sharp of femoral retored well. The distance of crispation is 0.6cm in average(0.4-1.1cm).Conclusion Triple pelvic osteotomy is an effective surgery which can improve the relationship between the acetab and the caput femoris. Also it can improve the joint function of articulation of hip. After triple pelvic osteotomy, little cases happened such as premature arthritis of the hip in earily time. All in all triple pelvic osteotomy is an effective procedure in the treat of the avascular necrosis of femoral head in children. |
