Objective :The study is to investigate the clinical efficacy of anterior cruciate ligament reconstruction by long fibular muscle tendon and hanlstring tendon under arthroscopy after the rupture of anterior cruciate ligament.Methods:From 2010 February to 2012 June, Our department implemented 38 cases of reconstructing the anterior cruciate ligament by the long fibular muscle tendon and hamstring tendon under arthroscopy. In these 38 cases, the long fibular muscle tendon group are 18 cases, and the hanlstring group are 20 cases. This paper analyzes on differences between the two groups of patients with general evaluation of the project, and postoperative Lachman test, Lysholm score and IKDC score to get the clinical efficacy,through the retrospective, using SPSS 17 statistical software for statistical analysis.Results:There are no significant difference between the clinical data of two groups of patients, including gender, age, injured joints, and time from injury to operation.There are no significant difference between the general evaluation items of the two groups, including hospitalized time, operation time,24 hours joint drainage after operation(P) 0.05).There are no significant difference between two groups patients by Lachman test (P) 0.05) before operation.There are no significant difference between two groups patients which are respectively six months and two years after operation(P) 0.05).As it concerns to Lysholm score, before operation, the mean value of long Fibular muscle tendon group was 46.5 ± 3.87; the mean value of the hanlstring group was 47.9 ± 3.26. Six months after operation, the mean value of long fibular muscle tendon group was 85.3 ± 3.76; the mean value of hanlstring tendon group was 79.9 ± 4.26. Two years after operation, the mean value of long fibular muscle tendon group was 90.5 ± 3.94. Meanwhile, the mean value of hanlstring group was 89.6 ± 3.69.According to the group of independent samples t test, Lysholm score of two groups before operation and two years after operation, respectively, have no significant difference (P) 0.05). But Lysholm score of sample groups in six months after operation has significant difference (P<0.05).This shows the Lysholm score of long fibular muscle tendon group is better than that of hanlstring group.When it comes to IKDC score, the mean value of the long fibular muscle tendon group before operation was 50.2 ± 4.61. Meanwhile the mean value of the hamstring group before operation was 51.3 ± 3.98; the mean value of the long fibular muscle tendon group six months after operation was 86.3 ± 3.76.The mean value of the hanlstring group six months after operation was 80.9 ± 3.26. The mean value of the long fibular muscle tendon group two years after operation was 92.3 ± 4.52. Meanwhile the mean value of the hamstring group two years after operation was 91.8 ± 4.02. According to the group of independent samples t test, by the IKDC score, the two groups before operation and two years after operation, respectively, have no significant difference (P) 0.05). But in six months after operation has significant difference (P<0.05). This shows the IKDC score of long fibular muscle tendon group ls better than that of hanlstring group.Conclusion:The technique of anterior cruciate ligament reconstruction using long fibular muscle tendon under arthroscopy has the advantages of convenient operation, curative effect.Its early knee function recovery and patient subjective satisfaction is better than the hamstring group.It can prevent further damage patient tissues around the knee.It can avoid early knee muscle strength decline, saphenous nerve injury, PES anserinus dysfunction, anterior knee pain.It has quick postoperative recovery.Also, do not appear for the tendon area complications obviously after resection of long fibular muscle tendon.Because the study of small numbers of cases, the follow-up time is not long enough, the lack of characteristics of evidence-based medicine multi center, large sample, randomized study series, so the use of long fibular muscle tendon for reconstruction of anterior cruciate ligament under arthroscope preliminary effect is good, still need future increase sample size, long time follow-up, to furtherly study the clinical curative effect of the operation and the effect on tendon area.
