Objectives To compare the effectiveness of anterior cruciate ligament(ACL)reconstruction using long fibular muscle tendon or hamstring tendon under arthroscopy. To analyze the relevant indexes before and after operation to determine more appropriate grafts and provide reference for clinical practice. Methods 81 patients with anterior cruciate ligament rupture were randomly divided into two groups.PLT and HT were used to reconstruct the anterior cruciate ligament, including 45 cases of PLT group and 36 cases of HT group,The International Knee Documentation Committee's knee joint assessment(IKDC score),Lysholm knee score and ankle score(kofoed score)were used to evaluate the function of knee joint and ankle joint Before and after surgery Subjectively,The Lachman test,ADT test and joint activity were used to evaluate the two groups objectively,The operation time, blood loss, intraoperative blood loss,24 hours postoperative drainage, hospitalization time were compared at the same time. Results All patients were followed up for 12 to 15 months with an average of 13.7 months. The Lysholm score of the PLT group was increased from(44.36±9.22)points before surgery to 3 months(75.36±8.07),6 months after operation(84.12±3.42),12 months after operation(91.72±3.88);The IKDC score of PLT group was increased from preoperative(53.16±3.84)to 3 months after operation(78.56±2.79),6 months after operation(86.48±3.65),12 months after operation(92.04±3.78)points;The Lysholm score of the HT group was increased from(44.69±8.45)points to(68.45±5.96)points 3 months after operation,6 months after operation(79.37±3.52),12 months after operation(90.49±4.77)Minute;The IKDC scores of HT group were increased from(52.74 ± 3.67)points to(69.63±5.68)3 months after operation,6 months after operation(78.34±4.44),12 months after operation(91.11±2.88)points.The distinction between the two groups before and after operation were statistically significant(P <0.05).The distinction between the two groups was statistically significant(P <0.05)at three months and six months after operation,There was no important difference between the two groups after 12 months(P> 0.05).There were 3 cases of grade ? relaxation and 1 case of grade ? relaxation in the PLT group and 4 cases of grade ? relaxation and 2 cases of grade ? relaxation within 12 months after operation. Conclusions 1 Compared with preoperative, the use of peronea long tendons or hamstring tendon for the reconstruction of the anterior cruciate ligament can significantly restore the function of the knee, and can achieve good clinical results;2 within six months after reconstruction, compared with the hamstring tendon,The application of anterior cruciate ligament with peronea long tendon can achieve better clinical outcomes.3 In the long term, the use of peronea long tendons or hamstring tendons can achieve good therapeutic effects, both of which are ideal graft materials.
