The Systematic Review Of Kappa Free Light Chains On The Diagnosis Values Of Multiple Sclerosis

Objective:By comparing system evaluation of Kappa free light chain in cerebrospinal fluid,KFLC index,Ig GOligoclonal bands,know how these factors affect the diagnosis value of multiple sclerosis.Methods:By searching Pubmed, Embase, Cochrance library,professional electronic database and manual retrieval,among them I have searched related literature about Kappa free light chain,KFLC index,Ig G Oligoclonal bands.Use the Research evaluation tool involved in the Diagnostic test system recommended by Cochrance network system in 2011 to evaluate the Quality of the literatures involved and use Meta Discl. 4 software to quantify,compose and fit SROC curve,then work out the area under curve and Q index.Results:By screening,seven related literatures are chosen.The datas that KFLC diagnoses Multiple Sclerosis are as follows:the sensitivity is 87.5%, Specificity is 97.5%,Diagnostic odds ratio(DOR)is 276.34,AUCSROC is 0.9831,Q index is 0.9433;the datas about KFLC are as follows:the sensitivity is 76.1 %, Specificity is 86.1 %,Diagnostic odds ratio is35.858, AUCSROC is 0.923,Q index is 0.856;the datas about OCB are as follows:the sensitivity is 90.4%, Specificity is 85.2%,DOR is101.81;the datas about Ig G index are as follows:the sensitivity is 67.7%, Specificity is 77.9%,DOR is 35.858,AUCSROC is 0.8141,Q index is 0.7482.Conclusion:By comparing the Kappa free light chain, KFLC index, Ig G Oligoclonal bands, Ig G index for multiple sclerosis, results suggest that KFLC has high accuracy to the diagnosisof multiple sclerosis, OCB is in the second place, the accuracy to the diagnosis of Ig G index is the worst, above all can provide evidence-based evidence for early clinical diagnosis of Multiple Mclerosis.Heterogeneous sources includes:research design and implementation-selected sex ratio, age, race, disease stage, set in the control group of the objectsin the study;different diagnostic gold standards;threshold differences; diagnostic technology anddifferences of operation methods, qpportunities and so on.After the quality evaluation of selected literatures, bias control of the literatures is better.Due to the selected people from Europe, the rest of the population was less, so the diagnosis may be suitable for European people.The results canprovide a basis for clinicians to diagnose Multiple Sclerosis and for experts to develop guidelines for Multiple Sclerosis.
