Background and Objective The vascular endothelium is a multifunctional organ with the function of maintaining vascular homeostasis, regulating cell proliferation and angiogenesis,preserving a non-thrombogenic blood-tissue interface. Studies have shown that inflammatory factors has important effects on vascular endothelial function. Endo-PAT2000 is a new, non-invasive vascular endothelial function detector. Endothelial function were assessed using the Endo- PAT2000 device in hypertensive patients,then observe the related factors of vascular endothelial injury in H type hypertensive patients. In this study the relationship between IL-6 and TNF-α levels and endothelial function in H type hypertensive patients was expeored.Methods From June, 2013 to Dec, 2013, we continuedly enrolled 130 hypertensives and 28 healthy participants from the second affiliated hospital of Nanchang university under the strict inclusion criteria. Patients are divided into H type hypertension(Hcy ≥10mmol/L, n=88), non-H type hypertension(Hcy <10mmol/L,n=42), mean age 59.94±10.47、58.12±10.13( P >0.05). Meanwhiee, included 28 healthy participants as control group, mean age 59.14±7.60. We also recorded age, blood pressure, history of smoking, fasting blood check high-sensitivity C-reactive protein(hs-CRP), blood lipid(total cholesterol TC, triglyceride TG, low density lipoprotein cholesterol LDL-C, high density lipoprotein cholesterol HDL-C, glycatedhemoglobin, homocysteine Hcy). In addition, The Endo-PAT2000(Itamar Medical Inc.Caesarea) noninvasivetesting equipment of vascular endothelial function measured RHI and AI which evaluate vascular endothelial function and arterial stiffness were operated in morning fasting adoption. Results Plasma concertration of IL-6 and TNF-α in H-hypertensives and non-H-hypertensives were significantly higher than healthy participants, IL-6: [11.47±7.94 pg/m L vs. 8.55±2.47pg/m L vs. 5.96±1.54 pg/m L, P <0.01, P <0.01]; TNF-α [(148.00±49.28) pg/m Lvs.(128.35 ±44.25) pg/m L vs.( 103.2 ±47.98) pg/m L, P <0.01, P =0.032]; H-hypertensives compared with non-H-hypertensives IL-6(P =0.048), TNF-α:(P =0.03), RHI in healthy participants were higher than H-hypertensives and non-H-hypertensives[(1.93±0.48)vs.(1.50±0.39) vs.(1.66±0.43), P < 0.01, P =0.008], H-hypertensives compared with non-H-hypertensives(P=0.048).The pearson correlation analysis indicated that, RHI is negatively associated with age(r=-0.233,P =0.003), smoking history(r=-0.184, P =0.021), NIL-6(r=-0.387, P <0.01)、TNF-α(r=-0.388, P <0.01), Hcy( r=-0.235, P =0.003), Lg(Creatinine)(r=-0.17,P =0.03), uric acid(r=-0.172, P =0.039), Lg(hs-CRP)(r=-0.158, P =0.048), hypertension classification(r=-0.164, P =0.04) but positively associated with HDL-C(r=0.171, P =0.032), AI(r=0.181, P =0.023)and AI@75(r=0.184, P =0.021). Multiple linear regression analysis showed that RHI were linearly associated with TNF-α, NIL-6, age, AI and hcy.Conclusion In contrast to experimental groups, RHI was higher in healthy participants than hypertensives, esspecially in H-hypertensives, but NIL-6 and TNF-α instead, indicate that H type hypertensives associated with more levels of inflammation and endothelial damage. RHI gradually declined with the elevation of IL-6, TNF-α, hs-CRP levels or the increase of age in hypertensives, and there is a linear correlation between them, which hinted that they may be associated with H type hyertensives endothelial injury. Further longitudinal studies are needed to better understand the causal relationship between endothelial function and inflammatory cytokines.
