Atrial fibrillation( AF) is one of the most common cardiac dysrhythmia encountered in clinical practice. So far, the mechanism underlying AF is not completely elucidated. There could be a lot of and complicated pathogenesis involved in the occurrence and maintenance of AF. At present, a lot of studies show that inflammation may be closely associated with AF. Inflammation may take part in the occurrence and maintenance of AF through the influence of cardiac electrical remodeling and structural remodeling. As the endogenous ligand of angiotensin II type 1 receptor related protein(APJ), Apelin is a novel cardiovascular active peptide discovered in 1998, and is involved in a lot of physiopathologic mechanisms with APJ. Recently there is an increasing amount of evidence to show that Apelin regulates inflammation in various tissues. At the same time it is reported that Apelin levels of lone AF subjects are significantly reduced. We do not know whether or not Apelintakes part in the occurrence and maintenance of AF through the regulation of inflammatory processes. In this study, we plan to observe the levels of serum Apelin and makers related to inflammation in persistent AF patients with valvular heart disease, and then to study on the relationship of AF 、Apelin with inflammation and the mechanism underlying AF.Objective To study on the relationship of serum Apelin level with inflammation in persistent AF patients with valvular heart disease and the mechanis m underlying AF.Methods 58 patients with valvular heart disease were selected, including 29 patients with persistent AF(AF group)and 29 patients with sinus rhythm(SR)(SR group).Compare the levels of serum Apelin and makers related to inflammation in two groups.Results Compare to SR group, Apelin and ejection fraction(EF)were significantly lower in AF group; while high-sensitivity C- reactive protein( hs-C RP)、interleukin-2(IL-2)、interleukin-6(IL-6) and left atrial diameter(LAD)were significantly higher in AF group; the results had statistical significance(P? 0.05). Correlation analysis showed that Apelin were correlated negatively with hs-CRP and LAD(r=-0.308,P=0.019;r=-0.313,P=0.017), and were correlated positively with EF(r=0.265,P=0.044).Conclus ion Serum Apelin level in patients with AF is significantly lower, and levels of makers related to inflammation are significantly higher. Apelin may be closely related to AF and inflammation, and Apelin may take part in the occurrence and maintenance of AF through the regulation of inflammatory processes.
