ObjectiveType 1 diabetes is the autoimmune disease caused by environment and heredity. The pathogenesis of type1 diabetes is virus, diet and other external factors induce autoimmune response of islet, especially those who have the genetic susceptibility. T lymphocyte mediated autoimmune responses was activation cause immune damage of islet beta cells, beta cell mass and function declined, leading to elevated blood glucose. The Th17 cell is CD4+T cell subsets, which involved in the pathogenesis of many autoimmune diseases. In this study, the peripheral blood Th17 cells ratio and serum IL-17 A, IL-23, IL-6,IL-1β levels were mesured in patients with type 1 diabetes, type 2 diabetes and healthy control group, comparison it’s expression difference among the three group. To understand the relationship between Th17 helper T cells and type 1 diabetes mellitus. MethodsAccording to WHO diabetes diagnostic criteria of 1999,20 patients with type 1 diabetes mellitus(T1DM),21 patients with type 2 diabetes mellitus(T2DM) and 22 healthy controls were enrolled in the study.The course disease of T1 DM about 6±0.3 months,an average age of T1 DM with 17±11 years old;The course disease of T2 DM about7±3.5 months,an average age of T2 DM with 38±8 years old.The Th17、Tc17 cell in peripheral blood were detect by Flow cytometry;The levels of serum fasting plasma glucose(FPG) was measured by glucose oxidase method; the fasting C-peptide(FCP) and fasting insulin(FINS)were measured by magnetic separation enzyme- linked immunosorbent assay; the levels of serum interleukin 17A(IL-17A)、IL-23、IL-6、IL-1β were measured by Enzyme-linked immunosorbent assay(ELISA).White blood cells(WBC)、neutrophils(N)、lymphocyte(L)、Alanine aminotransferase(ALT)、Aspartate transaminase(AST)、blood albumin(ALB)、globulin(GLB)、hemoglobin(HGB)、blood urea(BUN)and serum creatinine(Cr)were detected by biochemical method;All of the statistical data was processed with SPSS 17.0 software.Results1. There were statistically significant among the three groups,except WBC,N,L, GLB,ALB,AST,BUN(P﹤0.05);The course disease of T1 DM and T2 DM no statistically significant(P>0.05),but the age of T1 DM group was obviously younger than T2 DM group;BMI and fasting C-peptide level of T1 DM are lower than that of T2DM(P﹤0.001),FPG is higher than the T2DM(P﹤0.001);The level of BMI,SBP, DBP, HGB,ALB,Cr,TCH,TG in the T1 DM group were lower than that in T2 DM group and control group(P﹤0.05),the FPG was higher than that in T2 DM group and control group(P﹤0.05);The level of FCP,FINS in the T1 DM group were lower than that in T2 DM group(P﹤0.001),the Urine ketone body, Hb Alc,INS-Ab,GAD-Ab were higher than that in T2 DM group(P﹤0.05).2. The ratio of Th17 cells and the level of IL-17 A, IL – 23 and IL-1β in peripheral blood of patients with T1 DM were higher than patients with T2 DM and healthy controls group(P﹤0.001); The ratio of Tc17 cells in peripheral blood of T2 DM was higher than that in T1 DM and healthy controls group(P<0.02); But the level IL-6 was not difference among the three groups(P>0.05).3. The ratio of Th17 cells were positively correlated with the level of IL-17 A,IL-23 and IL-6 in the T1 DM group(P﹤0.001); The ratio of Th17 cells was positively correlated with the level of IL-17 A in the T2DM(P﹤0.05);But there were no correlated between the ratio of Th17 cells with IL-17 A,IL-23 and IL-6 in the control group(P>0.05).4. Multiple linear stepwise regression analysis show that the Th17 cells and IL-17 A were linear correlated with INS-Ab,the IL-23 and IL-6 were linear related to GAD-Ab,but the IL-1β was linear correlated with age.Conclusion1.The ratio of Th17 cells and the level of IL-17 A, IL-23 and IL-1β in peripheral blood of patients with T1 DM were higher than patients with T2 DM and healthy controls group.2. The ratio of Th17 cells was significantly positively correlated with the level of IL-23 and IL-6 in patients with type 1 diabetes peripheral blood. Which suggesting that IL-23 and IL-6 may be involveing in the T cell transformation to Th17 cells,and participate in the pathogenesis of type 1 diabetes.3. The Th17 cells and IL-17 A were linear correlated with INS-Ab,the IL-23 and IL-6 were linear related to GAD-Ab,Which indicated that Th17 cells and it’s cytokines may be involved in the pathogenesis of type 1 diabetes.
