[Objective]:Leprosy is a kind of chronic infection disease which caused by mycobact-erium, skin and peripheral nerve involvement in. The clinical manifestations are deli-nking eyebrow, nerve is bulky, hypopselaphesia, rhagadia manus and pedalis,ulcer, h-ypophalangism,claw-hand,carpoptosia,corneal ulcer,blindness etc,causing heavy bur-den to patients and their family whose physical and mental health are greatly influent by burden.Current common view is that the pathology of leprosy is a kind of thymus derived cell mediated dermatitis which is polygenic inheritance and caused by multifa-ctors.Its pathogenesis is complicated,the abnormity of CD+4T lymphocyte immunolo-gic function is the most important.The CD+4T lymphocyte is categorized into Thl and Th2.Th17cell,a new kind of CD+4T cell subpopulation is found recently,highly convey IL-17,and excerete IL-6and TNF-a.Th17has independent differentiate and growth regulatory mechanism which play an important role in immunological and inf-ectious disease.It is a hot topic for debate currently.At present there is no research report at home and abroad about the role of Th17cells and related factors IL-17in leprosy disease. In order to find the answer,we examine the expression of Th-17cell and IL-17on peripheral blood PBMC in patient,and compared to normal person,in order to study the role of Th-17cell and IL-17in leprosy.[Method]:Specimens come from The Second Affiliated Hospital of KunMing Medic-al College, Yunnan center for disease control and prevention (YNCDC),Skin diseas station of jinning county, leprosy village of ShuangBai county, Xishan and Guangdu CDC.The leprosy patients are51,the cured people are26, the leprosy reaction patients are5,and the control are56.We use the flow cytometry to test the Th-17cell and IL-17in four groups above mentioned. And use the SPSS19.0to analysis the results. [Results]:1. Th-17cells significantly raised in leprosy patients compare to normal persons.Th-17cells were significantly elevated in leprosy patients, the cured people and the leprosy reaction patients compared to controls (P<0.001).There was no statistical difference of the Th-17cells numbers between leprosy patients and the cured people (p>0.05),the number of Th-17cells in PB was more than that in MB,which showed a statistically significant(P<0.05). There was no statistical difference of the Th-17cells numbers between MB and PB in the cured people.2. IL-17and IL-6were significantly elevated in leprosy patients, the cured people and the leprosy reaction patients compared to controls (P<0.05).The sick group was significantly higher than the other three groups (p<0.05),and there was no statistical diffenence between the cured people and the leprosy reaction patients (p>0.05)3. The IL-17levels of leprosy patients was much higher in PB than that in MB, which showed a statistically significant(P<0.05). There was no statistical difference of the IL-17levels between MB and PB in the cured people.(p>0.05)4. There was no statistical difference of the IL-61evels between MB and PB neither in leprosy patients nor in the cured people (p>0.05). The IL-17level and IL-6level in the four groups had a positive correlation,the numbers of Th-17and the levels of IL-17in the four groups also had a positive correlation.[Conclusion]:1. Th17cells play an important role in the pathogenesis of leprosy.2Th17cells involved in the body's resistance to infection of Mycobacterium leprae process, and the generation of immune protection. By enhancing Thl cells mediated the cellular immune reaction, caused macrophages and other relevant immunity activity cell activating, limited or directly killed Mycobacterium leprae, also had a protection role to leprosy patients3.Th17cells secreted IL-17, IL-6to resist Mycobacterium leprae infection. IL-6participated in the pathogenesis of leprosy by promoting the differentiation of th17cells and the producing of IL-17.4. Although clinical symptoms have completely disappeared in the cured leprosy patients but the body's immune function is still not back to normal.
