Objective:The research was done to understand much about the clinical characteristics of Elizabethkingia Meningoseptica infection, the appropriate empirical anti-infection recommendation,and the risk factors of the poor prognosis, so as to raise up the practitioners’ awareness of diagnosis and treatment of the infection of the Elizabethkingia Meningoseptica.Methods:We retrospectively collected and analysed the epidemiology,the clinical characteristic,and the drug-sensitivity of 24 cases of the elizabethkingia meningoseptica infection from Fujian Provincial Hospital between January 2009 and December 2014.According to the prognosis,the 24 patients were divided into recovered group(14 patients,54.31±8.408 year) or deterioration group(10 patients, 77.70±3.578 year). The Logistic Regression statistical analysis was used to analyze the possible risk factors for the poor prognosis.The risk factors were sex,ICU(intensive care unit),coronary artery disease,diabete mellitus,COPD(chronic obstructive pulmonary disease),chronic heart disease,chronic kidney disease, cirrhosis,the exposure time of hospitalization,APACHEⅡ,polyinfection,use of broad-spectrum antibiotic,mechanical ventilation,HAP,aspiration,acute upper gastrointestinal bleeding.Results:We collected a total of 24 patients(74±22 year) of elizabethkingia meningoseptica infection,which were consisted of 14 male and 10 female patients.The patients were the ones who had many underlying diseases,such as coronary artery disease,hypertension,diabetes mellitus,and undergone the invasive procedure of mechanical ventilation or tracheotomy.The commom pathogens of polyinfection were acinetobacter baumannii, klebsiella pneumoniae, candida albicans and so on.All the cases of elizabethkingia meningoseptica infection were hospital acquired infection,which were predominantly manifested as HAP(87.5%) and sepsis(45.83%). The mortality of elizabethkingia meningoseptica infection was as high as 41.67%. The drug-sensitivity results showed that the elizabethkingia meningoseptica is sensitive to piperacillin-tazobactam and multi-drug resistant to carbapenems such as imipenem, mreopenem,and the fourth generation cephalosporin antibiotics of ceftazidime, cefepime,and aminoglycosides of gentamicin.According to the discharge outcome of every patient, the 24 patients were classified into the recovered group(n=14) and deterioration group(n=10).The department or the specimen distribution of the two groups showed no significant difference The risk factors,which included sex,ICU(intensive care unit),coronary artery disease,diabete mellitus,COPD(chronic obstructive pulmonary disease),chronic heart disease,chronic kidney disease, cirrhosis, polyinfection,use of broad-spectrum antibiotic,mechanical ventilation,HAP,aspiration,acute upper gastrointestinal bleeding, the exposure time of hospitalization, APACHEⅡ, were also not in significant difference.However,the ages,malignant tumor,and sepsis may be related to poor prognosis.Conclusion:The prevalence of nosocomial infection caused by elizabethkingia meningoseptica was sporadically distributed, and it was mainly found in the patients of ICU in.Fujian Provincial Hospital in the six years.Special attention of elizabethkingia meningoseptica infection should be made in the patients of ICU,HAP, sepsis,and multiple chronic underlying disease. Elizabethkingia meningoseptica was a multi-drug resistant bacteria and is inclined to be infected with other pathogens.The infection of elizabethkingia meningoseptica could cause high mortality,and the ages,malignant tumor,and sepsis may be the useful predictors of poor prognosis.
