Objective To analyze and compare the prognostic value of SOFA,qSOFA,APACHE III,and OASIS in patients with sepsis in intensive care unit(ICU).Methods Patients admitted to the ICU of Jingdezhen First People's Hospit al from December 2013 to December 2016 were enrolled in the study.Physica l examinations and some related clinical examinations were performed and reco rded within 24 hours after admission.SOFA,qSOFA,APACHE III,and OASI S scores were calculated according to instructions of each severity scoring syst em.ICU mortality was chosen as the primary outcome,and receiver operating characteristic(ROC)curves were performed to evaluate the prognostic value o f each severity score on the primary outcome.The areas under ROC curves(AUC)were calculated and compared between groups using the Hanley & Mc Neil method.Results A total of 279 sepsis patients were included in the study finally.The median length of ICU stay was 4.79 days,with an ICU mortality rate of18.28%(51/279).When compared with the survivors,non-survivors had highe r SOFA,qSOFA,APACHE III,and OASIS scores,all of which were statistic ally significant(P<0.05).ROC curves analysis showed that the AUC of SOFA,qSOFA,APACHE III,and OASIS scores in predicting ICU mortality in patie nts with sepsis were 0.725,0.594,0.758,and 0.801,respectively.Only the AU C of qSOFA was significantly different from the other three scores(P<0.05),while there was no significant difference in AUC between SOFA,APACHE II I,and OASIS(P> 0.05).Conclusion For patients with sepsis in ICU,the clinical values of differen t severity scoring systems on evaluation of prognosis were not consistent.The qSOFA scoring system showed a significantly weaker ability in predicting ICU mortality when compared with SOFA,APS-III and OASIS,while the other th ree scoring systems showed no significant difference.Considering the complexi ty of each scoring system,the OASIS score system might be more practical.
