The Effects Of Budesonide Nasal Drops On Postoperative Recovery Of Frontal Sinus

Objective:Observation of the effects of Budesonide nasal drops on postoperative recovery of the Frontal SinusMethods: 46 standard patients(61 nasal cavities)suffering from chronic frontal sinusitis were selected and underwent the surgery of endoscopic frontal sinus surgery(DraftⅠ~Ⅱ). Observation in the operation:Recording the Mucosal lesion severity、Sinus mucosa preserving degree、Sinus ostium opening diameter etc.,during the frontal sinus opening operation. The patients were grouped randomly matching according to the drop group and spray group in 1:1. Related risk factors should be ruled out. After the surgery the patients of the drop group were ordered to drip in the budesonide directly in Mygind position while the spray group using conventional posture. at the time of 2 weeks,1 month, 2 months and 3 months after the surgery, the endoscopic observations were carried out with the standard of Lund- Kennedy score system, recording the conditions about the polyps, edema, rhinorrhea, scar, crust around the frontal recess and ostium of frontal sinus. until the Lund-Kennedy score being ≤1, the epithelization was demonstated completed. The patency of the frontal ostium was evaluated endoscopically 1month,2months,and 3months postoperatively.it means fluent when the diameters was≥4mm,stenosis while that was< 4mm.Compare the compliance of the budesonide for patients, including the percentage of actual dose to the prescription dose、the frequency and dose for prescribing drugs when reviewing the case.lastly,make a Statistical analysis for the scores of the drop group and the spray group,compare the differences between the two groups.Result:1. The epithelization time of the drop group was significantly reduced, The epithelization time was between 50 to 100 days,the average was 90 days.83.9% of the patients have completed 3 months after the surgery.Only 33.3% of the spray group have completed the epithelization at 3 months after the surgery.The score of the drop group(0.97±0.84) was significantly less than that of the spray group(2.86±1.41), the P<0.05,in the Lund-Kennedy endoscopic score 3 month after the surgery.2.Compare the frontal recess 1month,2months and 3months after the surgery,the patency rate of the frontal sinus stium of the experimental group patients(83.9%、77.4%、77.4%) was significantly better than the control group(63.3%、60.0%、56.7%),p<0.05. and the experimental group(16.1%、19.4%、19.4%) had a lower incidence of stenosis than the control group(36.7%、33.3%、36.7%),but p>0.05,it showed no statistical significance.Conclusions: The Budesonide nasal drops in Mygind position would significantly reduce the epithelization time of the frontal sinus for the patients of chronic frontal sinusitis.
