Purpose:To explore the change law of the serum uric acid concentration in various degrees of male patients with obstructive sleep apnea hypopnea(OSAHS) and investigate the association between OSAHS and the serum uric acid. In order to do pilot study of whether male patients with OSAHS were predisposed to gout, cardiovascular diseases.Methods: A total of 104 male patients who underwent polysomnography in department of Respiratory of Zhong Shan hospital Xiamen university were enrolled between January 2012 to December 2014. According to the apnea hypopnea index from the results of polysomnography, all the subjects were classified into four groups: control group(AHI< 5/h, 8cases), mild OSAHS group(5/h≤ AHI ≤ 15/h, 19cases), moderate OSAHS group(15/h< AHI ≤ 30/h, 24cases), severe OSAHS group(AHI> 30/h, 53cases). To observe the difference of age, body mass index(BMI), neck circumference, waist circumference, ESS, blood urea nitrogen, creatinine, total cholesterol, triglyceride, blood uric acid and AHI, oxygen desaturation index(ODI), the percentage of total sleep time spent with Sp O2<90%(TST), lowest O2 saturation(La SO2), average O2 saturation(average Sp O2) among four groups. Appropriate statistical methods were used to compare the difference between each OSAHS groups and control group. Analysis of variance, One-Way ANOVA, LSD test, Kruskal Wallis test, Mann-Whitney test, Spearman correlation analysis and multiple linear regression analysis were used for statistics.Results: Compared with the control group, BMI, neck circumference, waist circumference, ESS in OSAHS group showed significant difference(P<0.05). Age did not show significant difference(P>0.05). The level of serum uric acid in mild OSAHS group(358.96±74.58)umol/L, Moderate OSAHS group(399.87±90.14)umol/L、severe OSAHS group(445.20±99.55)umol/L were significantly higher than control group(P<0.01). Compared to mild OSAHS group and control group, severe OSAHS group had a higher serum uric acid(P<0.05). Serum uric acid and AHI(r=0.436), TST(r=0.349), ODI(r=0.390), ESS(r=0.233), BMI(r=0.412), neck circumference(r=0.435), waist circumference(r=0. 309)were significantly positive correlation(P<0.05). Serum uric acid was negatively associated with the lowest oxygen saturation(r = 0.291), the average blood oxygen saturation(r = 0.244)(P < 0.05). Multiple linear regression analysis showed that AHI(adjusted R2=0.263, β=0.419, P=0.020), creatinine(adjusted R2=0.263, β=0.218, P=0.031) were the major contributing factors for elevated serum uric acid.Conclusions: OSAHS, renal insufficiency were importantly associated with elevated serum uric acid in male. The level of serum uric acid in male OSAHS patients was higher, had a positively correlated with BMI, neck circumference, waist circumference, AHI, TS90, ODI and had a negatively correlated with La SO2、average Sp O2.
