Objective:The present study found that the ocurrence of Diabetic nephropathy(DN) is associated with inflammation, cell factors, oxidative stress and other related factors.Most DN patients in the advanced stages of the disease mainly rely on dialysis to sustain life, according to the statistics of year 2012 China in dialysis registered primary disease in patients, there were 143145 cases, including 22217 diabetic nephropathy patients. accounted for 15.5%, ranks second after glomerulonephritis,patient need to spend 60000 yuan only a dialysis projectper yearand dialysis can lead to a variety of complications,hypertension, electrolyte disorders, malnutrition etc. It can be seen that there is a great need for the further study of diabetic nephropathy.Omentin is a newly discovered adipose tissue-specific secretory factor, and A close relationship with inflammation,oxidative stress,Metabolic disorders of glucose and lipid. It was reported that omentin-1 was correlated with chronic kidney disease without diabetic, but no relationship was reported between omentin-1 and diabetic nephropathy. We examined the level of omentin-1 in serum of type-2 DN patients and discussed the role of omentin-1 in type-2 DN disease, in order to provide theoretical basis for possible early diagnosis and monitoring of DN. Methods:1) object: 60 DN patients at nephrology Department and Endocrinology Department of second affiliated hospital of Shanxi Medical University, from June 2013 to October 2014 in hospital(III stage 30 cases, IV stage 30 cases), 46 newly diagnosed type 2 diabetes mellitus(T2DM) patients without renal damage at outpatient department of endocrinology and 44 healthy control group glucose patients of the same age and gender during the same time period.2) clinical index collection: record the general situation of the research object,Serum omentin-1 and biochemical parameters were determined. Results:There were no significant differences in gender, age and BMI between the three groups. In Newly diagnosed T2 DM group and DN group the FBG, Hb AIc, TG and HOMA-IR more significantly higher than the control group(P<0.05)(P<0.01), the levels of omentin-1 and LDL-C were significantly decreased(P<0.05)(P<0.01). The levels of BUN and Scr in the diabetic nephropathy group were significantly higher than that of the newly diagnosed type 2 diabetes mellitus group,the levels of FBG, Hb AIc, TG, FINs and omentin-1 in the diabetic nephropathy group more significantly lower than that of the newly diagnosed type 2 diabetes mellitus(P<0.05)(P<0.01). The Pearson correlation analysis, it was found that omentum- 1 was positively relate to HDL-C(r=0.413, P= 0.004) in newly diagnosed patients with type 2 diabetes mellitus, and HOMA-IR were negative correlation(r=-0.358, P=0.015). Serum omentum-1 levels and SCR、e GFR positive correlation(r=0.315, P=0.026)(r=0.349, P= 0.013) in diabetic nephropathy patients. Conclusion:Serum omentin-1 levels decreased may be one of the pathogenesis of diabetic nephropathy(DN), omentin-1 is one of monitoring indexes whether become type 2 diabetes, renal function of diabetic nephropathy, still need further study.
