Objective:Analyze and compare the clinical features of newly diagnosed young patients with type 2 diabetes,and provide clinical evidence for their prevention,diagnosis and treatment.Methods:A total of 521 newly diagnosed type 2 diabetic patients(?18 years old)who were hospitalized in the endocrinology department of the second hospital of jilin university from January 2015 to January 2018,317 males and 204 females,were divided into three groups according to age,and the young group(18-44 years old)249 cases,middle-aged group(45-59 years old)163 cases,old group(?60 years old)109 cases,compare the differences of the general and clinical data of three groups of patients,including gender,age,BMI,history of hypertension,family history of diabetes,smoking history,drinking history,blood ketone,FPG,2hPG,HbA1 C,UA,FCP,1hCP,2hCP,TC,HDL-C,LDL-C,TG,ALT,AST,fatty liver,proteinuria,diabetic peripheral neuropathy,diabetic retinopathy,and Ankle brachial index.SPSS21.0 software was used for statistical analysis,and P < 0.05 was considered statistically significant.Results:1?The proportion of male and obese patients in the young group was higher than that in the middle-aged group and the elderly group,and the prevalence of hypertension was lower than that of the middle-aged group and the elderly group.No statistical difference was found between the three groups in the family history of diabetes,smoking history and drinking history.2?The levels of triglyceride,alanine aminotransferase,fasting c-peptide andhigh uric acid in the youth group were higher than those in the middle-aged and elderly groups,and the concentrations of high-density lipoprotein cholesterol were lower than those in the middle-aged and elderly groups.The levels of glutamate transaminase,c-peptide 1 hour after meal and c-peptide 2 hours after meal were higher than those in the elderly group,while the levels of LDL cholesterol and glycosylated hemoglobin were lower than those in the elderly group,and the proportion of patients in the youth group with elevated blood ketone was higher than that in the middle-aged group.There were no significant differences in the levels of proteinuria,fatty liver disease,fasting blood glucose,blood glucose and total cholesterol 2 hours after meal between the three groups.3?The proportion of diabetic retinopathy,diabetic peripheral neuropathy and Ankle brachial index in the youth group was significantly lower than that in the middle-aged group and the elderly group,and there was no statistical difference in the proportion of proteinuria among the three groups.4?According to the multivariate analysis,the proportion of male patients and the abnormal proportion of uric acid in the young group were higher than those in the middle-aged group and the elderly group.The proportion of hypertension and diabetic retinopathy was lower than that of the middle-aged group and the elderly group;The proportion of abnormal blood ketone level was higher than that of the middle-aged group,and the ALT level was higher than that of the elderly group.The abnormal proportion of glycated hemoglobin and Ankle brachial index was lower than that of the elderly group.Conclusion:1 ? Newly diagnosed young adult people with type 2 diabetes have a high proportion of males and obese people.Diabetes screening for such people should be strengthened.2?Newly diagnosed young patients with type 2 diabetes are more likely to have metabolic syndrome,high uric acid,abnormal lipid metabolism(high triglyceride levels and high density lipoprotein cholesterol concentration)and impaired liver function(alanine aminotransferase and aspartate aminotransferase High)may berelated to the occurrence of type 2 diabetes in young people.3 ? Newly diagnosed young adult patients with type 2 diabetes have better pancreas islet function than middle-aged and elderly groups.4?The prevalence of hypertension and diabetic retinopathy in young patients newly diagnosed with type 2 diabetes is low,but it still exists.The risk of hypertension and retinal lesions in middle-aged and elderly patients is greatly increased.Attention should be paid to the monitoring of blood pressure and screening of retinal lesions in newly diagnosed type 2 diabetic patients in middle-aged and elderly groups.
