| Objective: Study the therapeutic effect on Dragon Bone and Oyster shell decoction for treat depression,and the chinese decoction effect on the HPA axis, by observing the patients of Qi depression transforming into fire type on change of HAMD-24 scale reductive ratio, as well as the change of fasting plasma CORT and ACTH in patients before and after treatment.Methods: There are 70 cases of Qi depression transforming into fire type were in accordance with the inclusive criteria. They were randomly divided into traditional Chinese medicine group and western medicine group. The Healthy control choose 30 health people in our hospital. The traditional Chinese medicine group used Dragon Bone and Oyster shell decoction for treatment. The western medicine group used Citalopram Hydrobro Tablets for treatment, 35 cases pergroup, all cases were treated for 8 weeks. Before treatment,after 4 weeks treatment and after 8 weeks treatment used HAMD-24 scale tested for the traditional Chinese medicine group and the western medicine group. Evaluating HAMD-24 scale reductive ratio to analyse the clinical total effective rate. At the same time, all cases draw blood must on empty stomach in the morning, and through Chemiluminescence test the change of content on CORT and ACTH. After 1 week treatment, after 4 weeks treatment and after 8 weeks treatment used TESS scale tested for the traditional Chinese medicine group and the western medicine group. Healthy control just draw blood once for test CORT and ACTH. Through the intra-group comparison for the traditional Chinese medicine group and the western medicine group before and after treatment tested for the change of HAMD-24 scale aggregate score, reductive ratio and TESs aggregate score. Through the intra-group comparison for the traditional Chinese medicine group and the western medicine group before and after treatment with healthy control observing the change of fasting plasma CORT and ACTH. Aanlysing the correlation between the fasting plasma CORT and ACTH and HAMD-24 scale aggregate score before and after treatment in traditional Chinese medicine group and the western medicine group. Through the inter-group comparison for the traditional Chinese medicine group and the western medicine group before and after treatment on HAMD-24 scale aggregate score, reductive ratio,TESs aggregate score and fasting plasma CORT and ACTH.Results: traditional Chinese medicine, western medicine group after treatment of 4 weeks, 8 weeks the scale aggregate score, TESS aggregate score and fasting plasma CORT and ACTH compare with before treatment had significant difference(P < 0.05); traditional Chinese medicine, western medicine group’s fasting plasma CORT and ACTH before treatment compare with Healthy control The two groups before treatment had significant difference(P < 0.05), and the after treatment had no significant difference(P > 0.05); HAMD-24 scale aggregate score was positive correlation with fasting plasma ACTH(r=0.68) and CORT(r=0.53); Through the inter-group comparison for the traditional Chinese medicine group and the western medicine group before and after treatment on HAMD-24 scale aggregate score, reductive ratio, TESs aggregate score and fasting plasma CORT and ACTH had no significant difference(P > 0.05); Two groups before and after treatment fasting plasma ACTH had had significant difference(P < 0.05); Two groups before and after treatment the TESs aggregate score had significant difference(P < 0.05).Conclusion: Traditional Chinese medicine group and western medicine group have quite clinical effect; Traditional Chinese medicine group and western medicine group before and after treatment the fasting plasma CORT and ACTH have changed demonstrate that the patients of Qi depression transforming into fire type have HPA axis’ s hyperfunction, and relates to the illness of severity, indicate that the Dragon Bone and Oyster shell decoction can play a role through effect on HPA axis. Traditional Chinese medicine group is better than western medicine group on side effect, traditional Chinese medicine group have fewer side effect and it’s safe for patients.
