Association Between H-type Hypertension And Premature Coronary Artery Disease

Objective:To explore the relationship between H-type hypertension and PCAD,and provide PCAD with clinical idea of early diagnose and treatment。Methods: 197 patients(male<55 years old,fermale<65 years old) with essential hypertension hospitalised in first affiliated hospital of Nanchang university were collected from 2012.12~2015.4。According to Framinham study,we define coronary artery disease with age(male<55 years old, fermale<65 years old) as premature coronary artery disease(PCAD)。All patients were detected homocysteine(Hcy),blood lipids and some other biochemical indicators levels 。 Personal details were collected,for example, history of smoking,and so on。Coronary lesion degree is calculated by gensini score。We separeted PCAD group(n=125) from all patients,the rest of patients are control group(n=72) according to coronary angiography。All the PCAD patients were divided into Acute coronary syndrome(ACS) group(n=35),stable angina grou(n=90) 。 Basis on coronary lesion all the PCAD patients were divided into single-vessel lesion group(n=64) and multi-vessel lesions group(n=61) 。Comparing H-type hypertension prevalence among different groups。With PCAD as dependent variables,H-type hypertension,drinking history and some other biochemical indicators as independent variables,the binary logitic regress analyzes their association 。 We analyze their association between H-type hypertension and PCAD。Results:1.H-tye hypertension morbidity,smoking and drinking history are obviously lower in the control group patients than that in the PCAD group(respectively 29.2%vs 76.0% P<0.001,23.6% vs 52.0% P<0.001,9.7% vs 21.6% P=0.034)。The male ratio,age,SBP,DBP,uric acid,LDL,HDL,TC and TG in the control group are not statistically different from that of the PCAD group(P>0.05)。The logistic regression analysis revealed H-type hypertension(OR=3.2) and smoking history(OR=3.0) are risk factors in PCAD。2.H-type hypertension morbidity,smoking and drinking history are obviously lower in the stable ungina group patients than that in the ACS group(respectively71.1% vs 88.6% P<0.001,40.0% vs 82.9% P=0.015,16.7% vs 34.3% P=0.032)。Age in the stable ungina group is apparently higher than that in the ACS group(54.5±6.9ages vs 49.6±8.2 ages P=0.001)。Multi-vessel lesions morbidity and Gensini score are obviously lower in the stable ungina patients than that in the ACS group(respectively 44.4% vs 60% P=0.003,40.2±10.6 vs 66.6±12.4 P=0.005)。The SBP,DBP,uric acid,LDL,HDL,TC and TG in the ACS group are not statistically different from that of the stable ungina group(P > 0.05) 。 The logistic regression analysis revealed H-type hypertension(OR=2.8) is a risk factor in ACS。3.H-type hypertension incidence rate and Gensini score are apparently lower in the single-vessel lesion group patients than that in the two or more than two vessels lesion group(respectively 67.2% vs 85.2% P=0.018,70.5±18.7 vs 25.6±7.5 P<0.001) 。 The male ratio,age,SBP,DBP,smoking and drinking history,LDL,HDL,TC and TG in the control group are not statistically different from that of the PCAD group(P>0.05)。In the two or more than two vessels patients,the logistic regression analysis revealed H-type hypertension(OR=2.8) is a predictor。Conclusion:1.H-type hypertension incidence rate in PCAD group patients is apparently higher than the control group;H-type hypertension incidence rate in ACS is apparently higher than the stable ungina group;H-type hypertension incidence rate in multi-vessel lesions group patients is apparently higher than the single-vessel lesions group patients?2.H-type hypertension is a risk factor in PCAD;H-type hypertension is a risk factor in ACS and multivessel lesions?...
