| Objective: To investigate the correlation between blood pressure variability(BPV)and the severity of coronary artery disease in patients with H-type hypertension complicated with coronary heart disease.Methods: From December 2019 to December 2020,100 patients with hypertension complicated with coronary heart disease were enrolled in the Department of Cardiology,Fourth Affiliated Hospital of Inner Mongolia Medical University.After obtaining all their consent,24-hour ambulatory blood pressure monitoring and coronary angiography were performed,and the plasma homocysteine(Hcy)level was measured.Patients with plasma Hcy≥10μmol/L were regarded as H-type hypertension group(H group),and patients with Hcy <10 μ mol/l were regarded as non-H-type hypertension group(non-H group).According to the results of coronary Gensini Score,H-type hypertension group was divided into mild group(Gensini Score<25 points),moderate group(25 points≤Gensini Score<50 points)and severe group(Gensini Score≥50 points).All of the selected patients were comparable in age,male ratio,body mass index,smoking history,diabetes history and blood lipid level.Compare the differences in Gensini Score and BPV between H group and non-H group,Compare the difference of BPV between different coronary lesions in H group,and analyze the correlation between Hcy and BPV and coronary Gensini Score.Results:(1)Gensini Score,24 h SCV,24 h DCV,d SCV,d DCV,n SCV and n DCV in H group are higher than those in non-H group,and there are significant differences in Gensini Score,24 h SCV,24 h DCV,d SCV,n SCV and n DCV(p < 0.05).(2)In H group,with the severity of coronary artery disease,24 SCV,24h DCV,d SCV,d DCV,n SCV and n DCV increase gradually,and the differences among the three groups are statistically significant(P < 0.05).(3)Correlation analysis shows that Hcy,24 h SCV,d SCV and n SCV in H group are positively correlated with Gensini Score(p < 0.05),while 24 h DCV,d DCV and n DCV has no correlation with Gensini Score(P>0.05).(4)Multiple linear regression analysis shows that when Gensini Score is taken as dependent variable and Hcy,24 h SCV,24 h DCV,d SCV,d DCV,n SCV and n DCV are all taken as independent variables,results are that Hcy,24 h SCV and d SCV are included in the regression equation,which are the influencing factors of Gensini Score.Among them,24 h SCV has the greatest influence.Conclusion:(1)Hcy and BPV are higher in patients with H-type hypertension complicated with coronary heart disease,as well as their more serious severity of coronary artery disease.(2)Hcy and BPV are the independent influencing factors of the severity of coronary artery disease,among which 24 h SCV has the greatest influence.(3)Hcy and BPV can be used as good indicators to independently predict the severity of coronary artery disease. |
