Objective To observe the dynamic change of the serum ferritin level and the perihematoma edema volume and explore the association of them in patients with intracerebral hemorrhage(ICH). The purpose is to provide the theoretical basis of assessment for cerebral edema, further to provide new idea to the prevention and treatment of secondary brain injury after ICH.Methods 80 cases of patients with ICH were employed and randomly arranged into conventional therapy group and minimally invasive treatment group. Conventional therapy group were given conservative treatment, and minimally invasive treatment group received frameless stereotactic surgery. The serum ferritin level and the perihematoma edema volume were recorded and compared between 2 groups at different time points(1、3、7、14d) after ICH.Results(1) In conventional therapy group and minimally invasive treatment group, the serum ferritin levels were higher than normal control group significantly. Compared with minimally invasive treatment group, the serum ferritin levels were higher at 3d, 7d and 14 d after ICH in conventional therapy group.(2) There was no significant difference observed in the perihematoma edema volume between conventional therapy group and minimally invasive treatment group at 1d and 3d after ICH. Compared with minimally invasive treatment group, the perihematoma edema volume were more at 7d and14 d after ICH in conventional therapy group.(3) There was no linear correlation between the serum ferritin level and the perihematoma edema volume at 1d and 14 d in conventional therapy group and minimally invasive treatment group. However, there was a positive correlation between them at 3d and 7d in 2 groups. Conclusions The serum ferritin can be an important indicator of assessment for cerebral edema after ICH. Compared with conventional therapy, minimally invasive treatment can cut down the serum ferritin level and the perihematoma edema volume obviously in patients with ICH.
