Research On AMT Dose Of Breast Conversed Surgery Patient During Postoperative Diotherapy

The routine treatment for breast cancer patient is surgery, chemotherapy,radiotherapy. Breast conversed surgery and postoperative radiotherapy is recommended therapy for early stage patient. Breast conversed surgery(BCS)and postoperative radiotherapy was reported to have the equal overall result with asterectomy surgery. Moreover, it may reduce the local recurrence rate,lead to longer survival life with better cosmetic appearance. Heart region was within the dose distribution during the radiotherapy so that radiation-associated heart disease(RAHD) may happen. The majority cases happened at the anterior part of heart in RADH. The dose and volunme reduction at the anterior part of heart may decreace the percentage of RAHD. 23 left breasr cancer patient were randomly selected for our research. The PTVs and OARS was contoured by one radiation oncologist.8 treatment plans were complete by one phycisist, including7 IMRT plans and 1 3D-CRT plan. The heart, AMT, LV, heart+AMT,heart+AMT+LV, heart+LV, AMT+LV were set as OARS. The total dose supposed to deliver is 50 Gy with 25 fractions. We compared the detail parameter of 8 plans, including maximal dose, minimal dose, mean dose, etc.The result shows the high does region of heart, LV and AMT shrinked in IMRT plans, while low dose region of them enlarge. The reduction effect was almost the same in heart+LV and heart+AMT. The right breast and right lung get better protection, while AMT was set as OAR. Therefore, AMT is recommened to be set as OAR instead of heart to be optimized in the plan caculation and evaluation.
