Objectives By making model of diabetes and mandibular fracture of Wistar rat animal,observing the effection of allen phosphonic acid sodium, simvastatin alone and combination on the procession of mandibular fractures healing of diabetes rats. The role and impact is discussed the application of both drugs for diabetes and mandibular fracture,to provide the basis of the experiments on animals for jaw bone fracture patients with diabetes screening drugs for clinical.Methods One hundred and forty-four Wistar rats were randomly divided into six groups:the fracture group(A),the fracture and diabetes mellitus group(B),the fracture and diabetes mellitus and insulin group(C),the fracture and diabetes mellitus and insulin and allen phosphonic acid sodium treatment group(D),the fracture and diabetes mellitus and insulin and simvastatin treatment group(E),the fracture and diabetes mellitus and insulin and allen phosphonic acid sodium and the simvastatin treatment group(F).After the success of the diabetes and mandibular fracture model is established for a week, medical intervention was given into corresponding group except of group A and group B. After 2weeks, 4 weeks, 6 weeks, 8 weeks the experimental rats were executed, determination the X-ray plates of the rat mandibular fracture zone, the changes of bone mineral density;Callus area HE staining and immunohistochemical; ELISA method for determination of serum bone protection element, tartrate resistant acid phosphatase, alkaline phosphatase and osteocalcin levels.Results 1 X-ray observation comparison:at 2 weeks X ray photos of all rats showed the fracture line is clear,uniform density is low.No significant differences between group and group;At 4 weeks,6 weeks,8 weeks group F fracture area density is relatively high,the fracture line gradually blurred.2 Bone mineral density:At 2 weeks comparison between each group had no statistical significance(P>0.05);At 4 weeks,6 weeks,8 weeks group F was obviously higher than that of group D and group E(P<0.05),there is statistical significance;Group D and group E was obviously higher than that of group C and group B(P<0.05),there is statistical significance.3 Histomorphology observation:at 2 weeks,4weeks,6 weeks in group F osteoclast number was relatively small,the number of osteoblast was relatively more.Osteoblasts were lined up on the surface of the bone matrix,formated the trabecular bone,and gradually transformed into bone cells.The trabecular bone fracture number,width and length was better than that of group D and group E;The number of osteoclasts of group D and group E was less.The bone trabecular number,width and length was better than that of group Cand group B.4 Immunohistochemical detection and The specificity of the TRACP dyeing:OPG and TRACP:At2 weeks,4 weeks,6 weeks,the positive expression of bone protection(OPG) and the positive expression of tartrate resistant acid phosphatase(TRACP) in group F compared with in group D was statistically significant(P<0.05);The positive expression of OPG and TRACP in group D compared with in group C was statistically significant(P<0.05);At 8 weeks there was statistical significance(P<0.05)compared with group B,comparison between other groups was no statistical significance(P>0.05);Osteocalcin: At 2 weeks,4 weeks,6 weeks the positive expression of osteocalcin in group F was higher than that in group E(P<0.05),the positive expression of osteocalcin in group E was higher than that in group C and group B(P<0.05), there is statistical significance;At 8 weeks there is statistical significance compared with group B(P<0.05),comparison between other groups was no statistical significance(P>0.05).5 Serum ELISA detection:OPG and TRACP:At 2 weeks,4 weeks,6weeks,the concentration of serum bone protection element and serum tartrate resistant acidphosphatase in group F compared with that in group D was statistically significant(P<0.05);At 8 weeks there was statistical significance(P<0.05)compared with group B,comparison between other groups was no statistical significance(P>0.05);Serum osteocalcin:At 2 weeks,4 weeks,6 weeks the concentration of serum osteocalcin in group F was higher than that in group E(P<0.05), the concentration of serum osteocalcin in group E was higher than that in group C and group B(P<0.05), there is statistical significance; At8 weeks there is statistical significance compared with group B(P< 0.05),comparison between other groups was no statistical significance(P > 0.05). Serum alkaline phosphatase:At 2 weeks,4 weeks the concentration of serum alkaline phosphatase in group F was higher than that in group E(P<0.05), the concentration of serum alkaline phosphatase in group E was higher than that in group C and group B(P<0.05), there is statistical significance; At 6 weeks, 8 weeks there is statistical significance compared with group B(P< 0.05),comparison between other groups was no statistical significance(P > 0.05).Conclusions 1 This study found that in diabetic rats mandibular fracture healing process,allen phosphonic acid sodium effectively increase the new callus and bone mineral density promote the expression of OPG and inhibit the release of TRACP, promote healing of fracture.2 It was found that simvastatin treatment of diabetic rats fracture can increases osteocalcin and ALP levels, promote healing of fracture.3 This study concluded that allen phosphonic acid sodium combined with simvastatin therapy of diabetic rats in fracture healing is superior to the single application allen phosphonic acid sodium or application simvastatin alone. Shows that the two drugs have better synergy for diabetic rats fracture healing.
