Object The purpose of this study was to investigate the analysis of ASTRAL Score to predict 1-month outcome in anterior circulation and posterior circulation acute cerebral infarction,and provide an objective basis for clinical treatment and prognosis assessment.Methods All consecutive patients hospitalized within 24 hours after onset of an acute cerebral infarction patients that were cured in Neurology department of our hospital and admitted by Magnetic Resonance Imagine were prospectively enrolled in the study.We applied the ASTRAL score to acute cerebral infarction patients admitted with in 24 hours.Related baseline date as the history of stroke and modified Rankin Scale before admission.The cut-off point of the study were defined as the 1-Month outcome(unfavorable outcome was assessed as a modified Rankin Scale score>2 at 1-months).The modified Rankin Scale score was used as the used as the golden standard to assess the prognosis of the patients. Patients follow-up was assessed during a central telephone structured interview or outpatients services at 1-Months.The predictive power of AC acute cerebral infarction and PC acute cerebral infarction was contrasted in ASTRAL score according to receiver operating characteristic curve(ROC curve).Result 1 There was significant difference in the ASTRAL score between favorable outcome and the unfavorable outcome of AC and PC acute cerebral infarction patients; 2The six project of ASTRAL score in favorable and unfavorable outcome groups shows different.In acute cerebral infraction group,Age,NIHSS,time delay between stroke onset and admission,acute glucose and level of consciousness were statistically different according to different outcome(P<0.05).And range of visual field defect was not statistical different in favorable outcome and unfavorable outcome.And in PC acute cerebral infraction group,the six project of the scale were statistically different in two groups(P<0.05); 3 The poor prognosis rate of AC and PC infarction patients shows an obvious rising trend with the increase of ASTRAL score;4 The AUC of the ROC curve for predicting a favorable outcome in patients with AC acute cerebral infraction(0.902)was higher than that in patients with PC acute cerebral infraction(0.788).The AUC of two groups was statistically different(P<0.05).For patients with AC infarction.the optimal cutoff score was 22,while with PC infarction patients was 19.Conclusions 1 ASTRAL score has good effect to predict prognosis at 1-months afteracute cerebral infarction patients; 2 ASTRAL score has a high predictive value on unfavorable prognosis of AC acute cerebral infarction.The predict value of ASTRAL score on AC infarction is higher than that on PC infarction.
