The Relationship Between Mental State And Pain Of Ultrasonic Scaling In Dental Patients

Objectives To investigate the relationship between mental state and the pain of ultrasonic scaling in dental patients,as well as the differences of periodontal index in different mental state patients before and after ultrasonic scaling.By analyzing the influence of different psychological states to pain degree in ultrasonic scaling and the periodontal condition,can guide the clinical doctors give patients different psychological counseling and treatment recommendations according to different psychological state, to let patients overcome psychological barriers, reduce the pain, in order to guarantees the smooth implementation of ultrasonic scaling, improve the rate of early see a doctor and regularly check of patients who has periodontal disease.Methods The subjects are the patients who were need to do the ultrasonic scaling in Beijing Coal General Hospital cadres Outpatient.The all selected subjects are unified into the standard accord with the experiment.Before doing the ultrasonic scaling,make the patient fill in the dental fear scale,self-rating anxiety scale and self-rating depression scale,in order to evaluate the psychological state,and then check and record their gingival index,simplified oral hygiene index and probing depth.When ultrasonic scaling to be done,use visual analogue scale to evaluate pain degree of treatment.After a week,arrange the patient to visit, check and record the patient’s periodontal index and VAS score again.At last,grouped the collected data, and do the statistical analysis.Results 1 The experiment total sent 205 questionnaires, recycled 205, recovery is100%.There are 95 men,and 110 women.Among them,dental fear patients accounted for29.3%,anxious state patients accounted for 19%,depression patients accounted for 22.4%.2 Different patients has different pain degree,but on the whole,the pain intensity of all the patients is in the medium level, the average score is 5.03 points. 3 By independent sample test and variance analysis, there are no significant differences about the degree of pain when doing ultrasonic scaling treatment in gender,age,education level, whether has ultrasonic scaling treatment history,whether feel pain and other dimensions. 4 By the statistical analysis,when doing the ultrasonic scaling,different psychological states patients can feel different pain degree.Patients who has fear,anxiety or depression mental state has higher pain degree than the normal,the difference is statistically significant. 5 There is a significant positive correlation between ultrasonic scaling pain degree and dental fearscore.The higher DFS score,the higher score of VAS. 6 Before ultrasonic scaling,the difference is statistically significant on the periodontal index of different psychological states patients.The average score of gingival index,simplified oral hygiene index and probing depth of patients who has fear,anxiety or depression mental state are higher then the normal. 7 After ultrasonic scaling,the average score of simplified oral hygiene index(include debris index and calculus index)of patients who has fear,anxiety or depression mental state are higher then the normal,but the difference is not significant,while the average of gingival index and probing depth are higher then the normal,the difference is statistically significant.Conclusions 1 when doing the ultrasonic scaling,different psychological states patients can feel different pain degree. Patients who has fear,anxiety or depression mental state has higher pain degree than the normal.The bad feeling will cause certain effects to patients’ body and mind,so dentist on clinical work should give full attention and reasonable explanation to patients’ with abnormal psychology,in order to reduce the patient’s fear and pain degree. 2 Periodontal condition is affected by the psychological state of patients to a certain extent,it is worse than the normal that patients who has fear,anxiety or depression mental state,so to be a doctor,we need to communicate with abnormal psychological patients reasonably,make proper psychological counseling to them, and guide them overcome psychological barriers,.make sure the periodontal disease can be early prevention, early diagnosis and early treatment. 3 After ultrasonic scaling,dentists should implement oral health education and regular review for patients, to master and control the periodontal recovery condition as far as possible.Give double attention to patients especially with negative psychological status, in addition to the regular orders,make them attache great importance to the cleaning and maintenance, promote the recovery of periodontal health.
