The Study Of Treating Of Children’s Femoral Neck Fracture By Double Incision Which Fixed By Hollow Screw Nails

Objective: Surgical approach based on double incision open reduction and hollow screw nails compression internal fixation method to treat children femoral neck fractures of the clinical efficacy of discussion, summarized by double incision surgical approach to treat children femoral neck fracture of the experience and lessons, femoral neck fracture surgery therapy for children add new ideas, so as to provide reference for clinical application.Method:Collected in June 2010- August 2014 in shandong university of Chinese medicine affiliated hospital pediatric orthopaedic ward, a total of 17 cases diagnosed as femoral neck fracture. 10 children of male, female, children’s seven. Age was 2-16 years old, with an average age of 9 years old. On the left side of the 10 cases, 7 cases is on the right. Fractures in Delbet classification. Ⅱtype 12 cases by the neck fracture. Type Ⅲbasal parts of the femoral neck fracture in 5 cases. All patients with imaging examination and assessment of soft tissue injury of scores determine the operation time, surgical treatment, intraoperative protect soft tissue, postoperative external fixation of plaster or support, guidance of clinical healing after dismantling external fixation of fracture in patients with functional rehabilitation, exercise and other measures to prevent the happening of the complications; Children 6 months to 5 years follow-up. Using yong-dong qiao the children put forward by the treatment of femoral neck fracture to evaluate curative effect evaluation standard.Results: Have follow-up results after treatment, 17 cases were followed up for 6 months to 5 years. 17 cases of surgery for stage I treatment of postoperative incision healing, wound infection and don’t heal phenomenon does not appear. Fracture achieved clinical healing and postoperative X-ray in fracture counterpoint good to the line review, internal fixation, no looseness fault happens, including 16 cases of surgical treatment of postoperative functional recovery is good, does not appear epiphyseal plate closure, early hip inside and outside the phenomenon of deformity, ischemic necrosis of femoral head. Only 1 case female postoperative 2 years a review of the appearance of ischemic necrosis of femoral head. Good evaluation optimal treatment in 16 cases, 1 case.Conclusion: Children femoral neck fracture is a relatively rare in children’s bone and joint damage type, double incision nail hollow screw compression internal fixation(orif) children femoral neck fracture has obtained more satisfactory clinical efficacy. Double incision surgical approach to treat children femoral neck fracture has the following advantages: 1. The small surgical trauma, low rate of skin flap necrosis; 2. Can be clearly revealed in the joint capsule, to look for of fracture end of anatomical reduction and fixation, reset the effect is good. 3. The little damage to the femoral blood supply. Late appear low incidence of the complications such as avascular necrosis of the femoral head.
