| Purpose: Through the study of ischemic cerebral apoplexy patients with class of traditional Chinese medicine,disease classification and serum homocysteine(Hcy) relationship,discuss the correlation,in purpose of providing objective indicators for the TCM syndrome type differences of patients with ischemic stroke,and providing evidence for evidence-based medicine clinical prevention of ischemic stroke.Methods: 127 patients confirmed by by skull CT or MRI diagnosis of ischemic cerebral apoplexy in Qilu hospital neurology ward in October 2014 to February 2015 are selected in this study.According to the Chinese medicine new medicine clinical research guiding principles of TCM standard enacted in 2002 and test results of the ischemic cerebral apoplexy syndrome classification statistics typing theses patients with Chinese medicine theory; Refer to ischemic stroke condition score sheet test results for patients with the disease score distinguish their disease category classification; Determine each patient’s serum homocysteine(Hcy) levels. Analyze the relationship between TCM syndrome, the condition score,disease category classification and serum homocysteine(Hcy) levels and investigate the correlation.Analysize the result datas using tatistical software of SPASS19.0 treatment.Results:(1)All patients in this study are divided into seven groups:Feng tan huo kang group(21cases 16.54%);Feng huo shang rao group(17cases 13.39%);Tan re fu shi group(13 10.24%); Feng tan yu zu group( 29 cases 22.83%); Tan shi meng shen grou(9 cases 7.09%); Qi xu xue yu group(20cases 15.75%); Yin xu feng dong group(18cases 14.17%). Descending order of proportion : Feng tan yu zu group, Feng tan huo kang group, Qi xu xue yu group, Yin xu feng dong group, Feng huo shang rao group, Tan re fu shi group, Tan shi meng shen group.(2)Analysize these 127 cases, 76 cases male, the proportion was 59.84%, 51 cases were female, the proportion was 40.16%, The proportion of male was slightly higher than female at the ratio of 1.5:1. The was no significant difference between between men and women(X2=6.830, P=0.337>0.05). The mean age was 61.60 ± 11.29 years. Comparise each TCM type group pairwisely there was no significant difference(P> 0.05).(3)The ratio of Hyperhomocysteinemia(HHcy) in each TCM group of the ischemic stroke ranged from high to low as following : Feng huo shang rao group,Tan shi meng shen group, Yin xu feng dong group, Feng tan yu zu group, Qi xu xue yu group, Feng tan huo kang group.The serum homocysteine(Hcy) levels of each TCM group ranged from high to low as following: Tan shi meng shen group, Feng huo shang rao group, Tan re fu shi group, Qi xu xue yu group, Feng tan yu zu group, Yin xu feng dong group, Feng tan huo kang group.Analysis by variance, the difference was statistically significant(F=2.807, P=0.014?0.05).The level of Feng tan huo kang group was lower than other groups, and the difference was statistically significant.(4)The serum homocysteine(Hcy) levels of ischemic stroke illness grade among light group, ordinary type group, heavy group and very heavy group were significantly different, with statistical significance(P?0.01). The differences between each groups were significant(P?0.01).Conclusion:(1)Feng tan yu zu group, Feng tan huo kang group and Qi xu xue yu group were three TCM groups that have a high frequency of occurrence. The most frequent representation were “Tan” and ”Yu”. This verified that “Tan” and “Yu” were the most common pathogenesis of ischemic stroke.(2)The incidence of ischemic stroke in male patients is higher than female.(3)The serum homocysteine(Hcy) levels of each TCM group ranged from high to low as following : Tan-shi-meng-shen group, Feng-huo-shang-rao group, Tan-re-fu-shi group, Qi xu xue yu group, Feng tan yu zu group, Yin xu feng dong group, Feng tan huo kang group. The level of Feng tan huo kang group was lower than other groups, and the difference was statistically significant. The ratio of Hyperhomocysteinemia(HHcy) was higher in “Tan-re” group and “Feng-huo” group, which illustrate that “Tan” and “Yu” can lead to inflammation.(4)Qi xu xue yu group and Feng tan yu zu group got high point in both disease category classification and disease condition score, and the condition is more serious if outbreaking; Feng tan huo kang group, Tan shi meng shen group and Yin xu feng dong group got low point in both disease category classification and disease condition score, and the condition is less serious if outbreaking; Feng huo shang rao group and Tan re fu shi group are between the two categories mentioned above.(5)The serum homocysteine(Hcy) levels was positively correlated with the illness grade of ischemic stroke. The serum homocysteine(Hcy) levels increases from light group to ordinary type group to heavy group to very heavy group.The level of Hcy parallels with the severity of the disease. |
