Objective:To compare the quality of life (QoL) of type 2 early period diabet ic nephropathy patients with typy 2 clinical diabetic nephropathy patients. Ana lysis of the related factors of the QoL. And analysis the risk factors of diabeti c nephropathy.Methods:From August 2013 to December 2014,a total of 310 patients diagnos ed as type 2 diabetes mellitus (T2DM) were enrolled in the study in endocrin ology^ diabetes and kidney internal medicine of the First Affiliated Hospital of Kunming Medical University. According to random urine albuminuria to urine creatinine ratio (UACR), patients were divided into pure DM group (normal albuminuria group=A group, n=106),early diabetic nephropathy group (microal buminuria group=B group, n=103) and clinical diabetic nephropathy group (mas salbuminuria group=C group, n=101). The group C is divided into C1 group and C2 group. According to the time of disease, the group C is divided into C3 group、 C4 group and C5 group.With SF-36 and KDTA scale were assessed to investig ate the quality of life of early diabetic nephropathy group and clinical diabetic nephropathy group. Analysis of relative factors for the quality of life of diabe tic nephropathy (DN). Clinic and laboratory date of all patients were collecte d:age, diabetes duration, hemoglobin, blood urea nitrogen, serum creatinine, blood uric acid, triglyceride, total cholesterol, fasting blood glucose, pre-albumi n, glycosylated hemoglobin AlC, serum albumin, homocysteine, Cysteine C and low density lipoprotein cholesterol,2-hour postprandial blood glucose, waist hip ratio, body mass index and analysis of risk factors for DN.Results: ①The aggregate score of the type 2 early period diabetic nephropathy group in SF-36 and the field of its branches is significantly higher than the c linical group, and the difference is statistically significant (P<0.05);The aggregat e score of the early group in KDTA and the field of its branches including SP L, EKD, BKD, WS, CF, QSI, SexF, Sleep is significantly higher than the clini cal group,the difference is statistically significant (P<0.05),but they have no stat istically significance in the remaining branch.②The QoL of two groups of T2D N patients were negatively correlated with age、DCD、BUN、Scr、TC、CysC、 LDL-C and positively correlated with HB、ALB.③There were statistically Signi ficant differences in DOD,HB,UA,FBG Logistic regression analysis showed that FBG、BUA and COD were the risk factors with T2DN (OR=1.298、1.008 and 2.976, P<0.05).Conclusion:The study shows the quality of life of the early group is higher t han the clinical group. The QoL of two groups of diabetic nephropathy patient s was negatively correlated with age、DOD、BUN、Scr、TC、CysC、LDL-C a nd positively correlated with HB、ALB. In order to improve the patients’QoL. We should pay more attention to these indicators in clinical work. Aclose corre lation has been found between FBG、BUA and DOD. So active management a nd clinical interventions for patients with early diabetic nephropathy, may slow down the progress of diabetic nephropathy. Monitoring and evaluating the risk factors will be helpful for prevention and treatment of DN.
