ObjectiveTo analyze the risk factors of diabetic nephropathy, for early prevention and better clinical treatment.Methods238 cases with type 2 diabetes admitted into the first hospital of China Medical University were enrolled into my study. To measure urinary albumin and urinary creatinine synchronously, three groups are divided according to ACR (albumin to creatinine ratio). There were 41 men cases with ACR<22mg/g,49 women cases with ACR<31mg/g of diabetes mellitus without nephropathy (normal albuminuria); 32 men cases with ACR 22-220mg/g,41 women cases with ACR 31-220mg/g of early-stage diabetes nephropathy (microalbuminuria); 33 men cases and 42 women cases with ACR>220mg/g of clinical-stage diabetes nephropathy (overt proteinuria) Age,gender,diabetes duration. Body height,weight,blood lipids (TG,TC,HDL-C,LDL-C),blood glucose (FBG,PBG),glycosylated hemoglobin (HbA1c),renal (Scr,BUN),serum uric acid (UA),plasma fibrinogen (FIB),urinary albumin and urine creatinine of each patient were recorded, calculate ACR and analysis.ResultsACR,FBG,PBG,UA,TG,HbA1c,FIB,BUN,Scr,DD were statistically significantly different among three diabetic subgroups(P<0.05); There are no statistical differences between the three subgroups in TC,HDL-C,LDL-C (P>0.05); DD,FBG,PBG,HbA1c,TC,TG,FIB,Scr,BUN,UA were positively correlated with ACR (P<0.05); DD,HbA1c,TG,FIB,UA are selected into the equation, and they were the independent risk factors of diabetic nephropathy.Conclusionslong duration of type 2 diabetes,hyperglycemia,dyslipidemia,high fibrinogen and uric acid are result in the development and progression of diabetic nephropathy. ACR regular testing will help early detection of DN. Positive control of blood glucose, correcting coagulation indexes,blood lipids and blood uric acid metabolism disorder is a better way for prevention of DN.
