The research background:Sudden deafness or sudden deafness,is a common disease of the ear. The etiology is not cleatr,the common clinical possible causes include:virus infection,inner ear microcirculation,metabolism,immune factors,cochlear window breakdown etc.The current research on sudden deafness,focuses on the young patients,and easy to overlook thesenile sudden deafness patients.China’s sixth census data shows that the population aged 60 and above accounted for 13.26%,2.93 percentage points higher than the 2000 census,the population aged 65 and over accounted for 8.87%,1.91 percentage points higher thanthe 2000 census.Changes in the age structure of the population inour country,along with the rapid developmentof economy and society in China,the great improvement of people’s living standards and health care,the acceler ated process of population aging.Sudden deafness in the elderly is over the age of 60 patients with sudden deafness.and the incidence rate in recent years has increased year by year and the pathogenesis is more complex,and the physiological function of elderly patients with diabetes, hypertension, hyperlipidemia, arteriosclerosis, and other relat ed diseases,increased blood viscosity,lowelastic factors of vascular microcirculation disorder is more common,so the problems ofrehabilitat ion, poor curative effect,high disability rate,and most of the elderly concept thatdeafness is the normal perform ance of old age,there is no need for treatment,so as todelay the best treatment time.Objective:This paper retrospectively analyzed the General Hospital of PLA Hospital treatment of sudden deafness in the elderly patients with clinical data, to explore the relationship between the related factors of factors, features and prognosis, focuses on the analysis of the related factors of the disease, and to observe the therapeutic effect of the relationship between the various relevant factors, and influence on the prognosis of the patients, so as to provide reference basis for clinical prevention and treatment of senile sudden deafness.Methods:On 2008 July to 2013 November in Chinese PLA General Hospital for treatment of 159 cases of sudden deafness in the elderly patients with complete clinical data of the data, the incidence, duration, gender, age composition, onset inducement, accompanying symptoms, audiological examination, blood tests, imaging examination, treatment method and effect retrospective statistics and analysis, the study of risk factors affecting the prognosis.Results:1.The collected 159 cases of more than 60 years old senile sudden Deafness cases,accounting for the same period were all patients with sudden deafness(1285 cases)12.37%.88 males,71 females(55.35%)(44.65%),the proportion of men and women= 1.24:1.Age of onset in 60-65 years old(64),followed by the 66-70 age(43),with the increase of age,the incidence of at least over 76 years of age(20),the average age of 67.9 years.2.There were 15 cases of patients with definite causes,of which 6 cases of cold history,5 cases of excessive fatigue,noise in 3 cases,1 cases of diarrhea.In 159patients, 101 cases(63.52%) accompanied by tinnitus,44 cases (27.67%)accompanied by vertigo.3.According to the time after the onset of treatment group 73 cases Of less than 7 days,86 cases of not less than 7 days.4.Grouped according to the degree of hearing loss:8 mild cases (5.03%), moderate in 33 cases (20.75%),54 severe cases (33.96%), severe 64 (40.25%).5. According to the hearing curve type grouping,to total deafness type in 71 cases (44.65%)and flat type in 53 cases (33.33%).6. Blood tests, including abnormal white blood cell count in 22 cases(13.84%); cytomegalovirus antibody detection, of which 9cases of abnormal (5.66%).31 cases (19.50%) glucose abnormalities,74 cases (46.54%) patients with essential hypertension.7.Accompanying diseases, with type 2 diabetes are31 cases (19.50%) of 159 patients; with hypertension are 74 cases (46.54%).8. Auxiliary examination,148 cases of abnormal ECG (93.08%);CT MRI, temporal bone and internal auditory canal water imaging examination in 159 patients showed no abnormality;carotid artery B ultrasound examination,including 88 cases of carotid atherosclerosis and plaque (55.35%). Vertebral artery B ultrasound examination in 159 cases, including 5casesof carotid atherosclerosis and plaque(3.15%).9. All patients treated with effective rate of 62.89%. The factors affecting the prognosis,efficacy of age, degree of hearing loss, hearing curve type, ECG, there were no significant differences(P>0.05);comparison of curative effect of the treatment time,whether with vertigo, vestibular function, carotid artery ultrasound, whether patients with type 2 diabetes and hypertension, the differences were statistically significant(P<0.05).Conclusion:1. Elderly patients with sudden deafness,no significant difference in incidence between men and women. Prognosis was no correlation with age2. Elderly patients with sudden deafness often accompanied by presbycusis, the degree of hearing loss was severe, which was mainly very severe and severe, The main types of hearing curve is complete deafness and flattened.3. The total effective rate in treatment of elderly patients with sudden deafness is high, the cure rate is low.4. The prognosis of elderly patients with sudden deafness is associated with treatment time, whether with vertigo, vestibular function and carotid artery,and is irrelevant with the degree of hearing loss, tinnitus and hearing curve type; with the increase of mean age associated diseases diagnosis, the proportion of concomitant diseases was significantly higher, it is found that the higher the age, the more the incidence of patients,the worse the prognosis.these diseases may affect the inner ear vascular function and blood circulation, effect of deafness prognosis.5. For the elderly sudden deafness, the major obstacle of improving the elderly hearing abilities is the neglection of deaf phenomenon, The conventional wisdom is that hearing loss is a normal manifestation of age, treatment is not necessary, and delay the best treatment time, it should arouse the attention of patients and medical workers.6.The treatment of sudden deafness in the elderly should be taken therapy comprehensive treatment program, in particular, according to the physical laws of elderly patients and a variety of concomitant diseases, importance of early prevention, regular inspection hearing, timely treatment.
