Objective: Preliminary study The epidemiological and clinical characteristics of patients with breast cancer in The tumor hospital of Yunnan Province from January of 2004 to December of 2013, To provide some basis and suggestions for prevention and treatment of breast cancer in Yunnan Province.Methods:Retrospective analysis of 3879 patients from January 2004 to December2013 in Tumor Hospital of Yunnan Province for the first time in diagnosis and treatment of breast cancer patients with clinical and pathological conditions and to groups of different age groups and different ethnic groups, different cultural background data were epidemiologically related analysis, preliminary to the epidemiological and clinical characteristics of breast cancer patients in Tumor Hospital of Yunnan Province.Results:(1) General characteristics:A total of 3879 cases of breast cancer collected data, The age of onset was 21-82 years old, the average age was 47.96 ± 9.70 years, The median age was 47 years, Mainly concentrated in the age of 36-50 years; the incidence in women most, accounting for 99.5% of the total number, The majority of premenopausal patients, accounting for 66.1%; married investigation accounting for 99.3%; Education for middle school, the number of patients is the gigest accounted for 35.0% of the total number, followed by high middle school accounted for 30.0%, school and below accounted for 23.6% of the total number, technical secondary school and above accounted for 11.4%.(2) Clinical features:The investigation object of mass position as The out upper quadrant of the breast, in 54.8% of the total,Of which 1077 cases of left breast, right breast in 1050 cases,The total incidence of left breast cancer was 2094 cases,1785 cases of right breast cancer;Small tumor proportion is not high, the size≤20mm of the tumor accounted for only 12.4%,The highest is the size 21-50mm of the tumor, accounting for 59.9%,The second is> 50mm, accounting for 27.7%;The largest number of respondents in phase Ⅱ, accounting for 62.1% of the total,phase Ⅲ 23.1%, phase 0+Ⅰ 12.8%, phase IV2.0%; Number of people receiving neoadjuvant chemotherapy for 40.06% of the total number; Operation with modified radical mastectomy, accounting for 79.2%,The second total mastectomy accounted for 11.2%,Breast conserving surgery 9.2%. effect a radical cure surgery 0.4%, The number of accepted breast reconstruction operation accounted for the total number of 2.3%, Sentinel lymph node biopsy 10.7%; 0 metastatic lymph nodes patients in 24.3%,1-3 metastatic lymph nodes patients in 29.1%; 4-9 metastatic lymph nodes patients in 33.4%; metastatic lymph nodes patients in 13.2%;(3) Pathological features:most of them is Run non special cancer 87.4%, Secondly, non invasive carcinoma 6.9%, The particularity of invasive cancer 2.6%, Early invasive carcinoma 2.2%, Other rare cancer 0.9%; ER positive accounted for 62% of the total, accounting for 38% of the total negative; PR positive accounted for 60.8% of the total, accounting for 39.2% of the total negative; Low expression (Ki-67< 14%) accounted for 23.9%, high expression (≥14%) accounted for 76.1%; HER-2 amplification rate of 14.4%; molecular typing in luminal B accounted for the highest proportion, was 58.1%. Secondly, luminal A 20.4%, HER-2 over expression was 13.1%, basal like type was 8.4%;(4) Different age groups between ER, PR, pathological type, age at menarche, HER-2, staging and lymph node metastasis rate among statistical differences (P< 0.05);(5) Between different ethnic groups, PR, tumor size, ER stage and lymph node metastasis rate had statistical difference (P<0.05);(6) Different cultural level between the age of onset, pathological type, HER-2 staging and there were statistically significant differences (P<0.05);Conclusion:(1) Breast cancer in Yunnan Province, the incidence of overall assumes the trend of escalation, age of onset 21-82 years, median age is 47 years old, the age of onset mainly concentrated in 36-50; ncidence in women and premenopausal patients are in the majority, occupational distribution to farmers accounted for the highest proportion of cultural level is generally low, the incidence of minimum age of only 21 years old, we will work for the census may need to pay attention to the younger crowd.(2) The left breast disease incidence rate is higher than the right side, and the tumor location in most of outside the upper quadrant. Small tumor proportion is not high, in clinical stages Ⅱ, Ⅲ, Surgery with modified radical mastectomy, mainly, breast conserving surgery rate is low, the reconstruction rate low, neoadjuvant chemotherapy rate is low, in order to change this situation, we also need to do the work in many aspects;(3) Pathology type lubrication non specificity of the majority, ER and PR in patients with positive proportion with the national level consistent, molecular typing with Luminal type;(4) A trend of younger age at menarche.36-50 age group the incidence of most, but early invasive carcinoma and preinvasive carcinoma together ratio, er, PR positive rate, stage 0、Ⅰ and Ⅱ combined ratio is other group high;≥66 years old age group is the minimum number of onset, but the ratio of IV, lymph node metastasis ratio greater than 10 is the highest;(5) Minority may be a factor affecting the development of breast cancer.(6) Education may be a factor affecting the occurrence and development of breast cancer.
