Objective: The analysis of the effect by after through transpedicle cuneiform osteotomy joint internal fixing with screws for correction of uncompensated sagittal imbalance due to thoracolumbar kyphosis.Methods: Retrospective analysis was carried out and the cases detected in 2013.3-2015.3 in the Department of Spine Surgery of the Second Hospital of Dalian Medical University had operated on 8 patients who were thoracolumbar kyphosis by after through transpedicle cuneiform osteotomy joint internal fixing with screws. Colleted the 8 patients preoperative and postoperative X ray data, measured the related indicators of sagittally balanced such as TK、LL、PI、PT、SVA;Sagittal Cobb angle;examination of improved JOA;ASIA nervous functional classification;examination of VAS; postoperative appearance、the improvement of both life and work and the occurrence of complications. The average follow-up time was 4.8±2.7 months(from 2 month to 7 months) by making call or out-patient follow-up.Results: All the 8 patients were operated remedial surgery, the operation time was from 2.5 to 5 hours, averaging 3.5 hours. The bleeding during operation was about 600 to 2600 milliliters, averaging 1600 milliliters. There’s no death、paralysis and infection after operations. The ache of the patients in both waist and back had been alleviated or even vanished. They were all satisfied with the postoperative appearance. The quality of life had been improved apparently after receiving the surgeries. Compared with pre-operation, the postoperative TK angel did not change significantly. Simultaneously, the postoperative of LL angle has received ideal correction, which is fluctuated in PI±9°. The postoperative of PT angle has also received ideal correction, averaging 16.5°. The postoperative Cobb angle of all the patients had reached 32.0±9.51°.The examination of JOA and VAS of the followed-up patients had debased while the ASIA nervous functional classification and postoperative appearance and quality of life had been improved apparently. What’s important, the occurrence of complication was very low.Conclusion: Since we have found that this kind of surgery is proved to be safe for the fewer operation bleeding、shorter operation time and lower rate of complication、no lost for followed-up angle and the reliable effect, it is a kind of really safe effective reliable surgery to cure uncompensated sagittal imbalance due to thoracolumbar kyphosis.
