A Clinical Research On Different Oxygen Atomization Inhalation Methods’ Effects To The Patients With Acute Exacerbation Of Chronic Obstructive Pulmonary Disease

ObjectiveObserve and analysis the treatment effect and adverse reaction of three different methods of oxygen atomization inhalation for AECOPD patients, inorder to find out the most suitable method of oxygen atomization inhalation for AECOPD patients.Then the results will guide clinical nursing work.Method1. Research subjects select:According to the inclusion and exclusion criteria formulated by this study, collect 120 cases of AECOPD in-patients who received oxygen atomization inhalation in department of internal medicine in a tertiary hospitals,in Jiangsu Province.2.Groups and methods:The patients were randomly divided into the continuous inhalation group, intermittent atomization, atomization group has three groups randomly,40 cases in each group.All the patients are given the same routine treatment and nursing care, use the same dosage form and the same dose of inhaled drug for oxygen drive atomization inhalation treatment.Continuous atomization group: Combined with salbutamol sulphate and budesonide for oxygen inhalation for 15- 20 minutes,twice a day.Intermittent atomization group:Combined with salbutamol sulphate and budesonide for oxygen inhalation 10 minutes, pause for 10 minutes, then continue atomization inhalation, twice a day.Orderly atomization group: First to oxygen atomization inhalation with salbutamol sulphate for 10 minutes, pause for 5-10 minutes, then continue atomization inhalation with budesonide, twice a day.3.Observation items: Patients’ body temperature, pulse rate, respiratory rate, blood pressure, finger pulse oxygen (SPO2) of the first oxygen inhalation treatment are observated and recorded before treatment and lh after treatment. Breathing difficulties, cough grade classification and expectoration difficulty, the sputum character, changes of pulmonary rales, adverse reaction and atomization inhalation are observated and recorded before the treatment and 3 days after continuous treatment. Then all these are used to have a statistical analysis.Result1. The baseline assessment of patients investigated before treatment, with general information, vital signs, finger pulse oxygen, blood gas analysis, symptoms and signs, atomization inhalation treatment situation has no significant difference (P>0.05).2. The pulse rates of Orderly atomization group at lh after treatment were lower than those before treatment. Moreover, the pulse rates of Orderly atomization group were lower than Continuous atomization group at lh after treatment. The differences have statistically significances (P<0.05). The respiratory frequencies of Intermittent atomization group and Orderly atomization group at lh after treatment were lower than those before treatment. The respiratory frequencies of Orderly atomization group and Itermittent atomization group were lower than Continuous atomization group at lh after treatment. The differences have statistically significances (P<0.05). Three groups’SPO2 at lh after treatment were increased than those before treatment. SPO2 of Orderly atomization group is higher than that of continuous atomization group and intermittent atomization group at lh after treatment, there was statistical significant differences (P<0.05). Body temperature, systolic blood pressure and diastolic blood pressure of three groups has no difference (P>0.05).3. Breathing difficulties grades, cough grades,expectoration difficulty, sputum character, and pulmonary rales of three groups reduced 3 days after continuous treatment than those before treatment. Moreover, the breathing difficulties grades, and pulmonary rales of Orderly atomization group 3 days after continuous treatment, based on the group mean rank, were lighter than intermittent atomization group, and both lighter than continuous inhalation group. The difference have statistically significances(P<0.05).4. The incidence of mist clammy feeling and irritating cough incidence of Orderly atomization group were lower than that of Continuous atomization group and Intermittent atomization group; The incidence of palpitation and aggravating of chest tightness and shortness of breath in Orderly atomization group is lower than Continuous atomization group. The differences have statistically significances(P<0.05).ConclusionThe method of oxygen atomization inhalation ordered by different kinds of drugs and has a intermittent, can promote the stability of pulse rate and respiratory rate, improve patients’SPO2, and can effectively improve the clinical symptoms of AECOPD patients, reduce the incidence of adverse reactions, which is worthy to applicate and promote in clinical.
