PurposeThe research was designed to find out the conditions of postpartum depression, which is known as PPD, in Shandong Province, in order to analyze the relationships between PPD and its influencing factors from different aspects, e.g. biology, psychology, and social support. So as to provide references for the prevention and treatment of PPD in the future work in clinical practice, and also to give advices for mental adjustment of puerperal women and the development of physical and mental health of infants.Methods1. During the period from Nov. 2013 to Oct. 2014, trained investigators did the investigation and assessment work by investigating 315 puerperal women in a Maternal and Child Health Hospital in a city in Shandong Province, among which, only 302 puerperal women were filtered out under the research conditions and their medical records information were collected in accordance with the requirement of the research. In total, 296 shares of valid questionnaires were collected. Therefore, the number of study subjects is 296 puerperal women.2.According to statistics, 296 puerperal women who had given birth between 7 to 42 days took part in the investigation. Social support conditions were measured using the Social Support Rating Seale, and mental conditions were assembled using the Edinburg Postnatal Depression. For those puerperal women who got a total score of EPDS higher than 13 points are considered as PPD positive, and their physiological, psychological and social support factors are taken into deeper analysis. Of all the subjects of the study, there are 75 cases of PPD patients, who are distinguished as Study Group, while the other 221 cases of non-PPD patients are distinguished as Control Group.3. All data are organized into descriptive statistical analysis using SPSS22.0, in categories like x2-test, t-test and multiple factor non conditional logistic regression analysis. The value of P is less than 0.05; therefore, the conclusion has statistical significance.Results1. In the survey, 302 puerperal women signed in the informed consent form and filled in the questionnaire, in which 296 were considered valid questionnaires, making the efficiency 98%. Study of the 296 cases detected 75 cases of postpartum depression, accounting for 25.3% of the total study subjects.2. Through comparative analysis, there is no statistically significant difference between factors concerning residence, education, occupation, economic income, parity and feeding method. On the contrary, factors concerning the age, maternal health care knowledge, depression during pregnancy, husband’s expectation for male baby have significant differences(P<0.05) between the PPD group and the non-PPD group. Through the single factor, non conditional multivariate logistic regression analysis, is can be concluded that the PPD social influence factors include age, pregnancy anxiety and depression, husband’s expectation for male baby, negative life events, and lack of social support, and so on.Conclusions1. The incidence of PPD in the city, which is 25.3%, is relatively high.2. PPD is the result of comprehensive effect of physiological, psychological and social factors. Age, anxiety and depression during pregnancy, negative life events, lack of social support, and the dashed expectation for male infant are high risk factors of PPD.3. It is an important mission for maternal and child health care institutions to take active measures, in accordance to the high risk f actors, to reduce the incidence of PPD.
