Objective: Sepsis whose disease develop rapidly and mortality rate is high, there is no effective therapeutic methods, it is still a problem in the current. Some studies show TGF-β1 as an immunosuppressive factor, participate in regulating sepsis. But its effect is controversial, mechanism is still obscure. The study compare amounts of TGF–β1 in peripheral blood between different severity of sepsis patients, built rat model with sepsis by cecal ligation puncture, record changes of TNF-α and IL-10 in peripheral blood about different teams, tried to find how TGF-β1 to regulate inflammatory response and mechanism of sepsis.Methods: Clinical investigation: According to the diagnostic criteria for sepsis and severe sepsis, collected sepsis and severe sepsis cases who were in emergency intensive unit from June 2014 to December 2014. Cases must be confirmed, and be collected serum samples of the day in hospital and the first day and the third day and fifth day and the seventh day and the fourteenth day. Detecting concentration of TGF-β1 in serum. Animal experiment: A total of 100 adult male SD rats whose weight were between 220 g to 250 g were randomized into 3 groups as follows : normal control group(n=4), CLP group(n=16) and TGF-β1 intervention group(n=80). These 80 rats of TGF-β1 intervention group were randomized into 5ng/(ml·250g) group,10ng/(ml·250g) group, 20ng/(ml·250g) group,50ng/(ml·250g) group,100ng/(ml·250g)group according different concentrations. Each groups had 16 rats. CLP group and each TGF-β1 intervention group were randomly selected 4 rats to collect blood at sixth hour and twelfth hour and twenty-fourth hour and Forty-eighth hour. Built rat model with sepsis by cecal ligation puncture, and injected in ringer’s solution 5 ml in abdominal cavity. CLP group would be injected 1ml NS at 0.5h after CLP, TGF-β1 intervention group would be injected TGF-β1. Detecting changes of TNF-α、IL-10 in serum every groups, then killed rats and collected Lung and liver to make HE slices in order to view the morphological changes. Analysis the data by SPSS20.0, compared the differences in each group. When P<0.05, it’s Statistically significant.Results: TGF-β1 of severe sepsis group is higher than sepsis group, sepsis in patients with peripheral blood TGF-β1 content associated with sepsis severity. Sepsis is more serious, TGF-β1 is higher. The concentration of TGF-β1 is higher, IL-10 is higher. The higher concentration of TGF-β1 can improve concentration of IL-10. Different TGF-β1 intervention groups lead to different TNF-αlevel. When concentration of TGF-β1 is 50 ng/(ml·250g), level of TNF-αis going up. That means it can promote TNF-α. When concentration of TGF-β1 is 100 ng/(ml·250g), level of TNF-αis falling. That means it can decrease TNF-α. Comparing morphological changes in different TGF-β1 intervention groups at twenty-fourth hour. When TGF-β1 is little, kidney and lung cells infected by inflammatory cells are more serious in CLP group, tissue necrosis is also obvious. However kidney and lung cells infected by inflammatory cells are better.Conclusions: TGF-β1 of severe sepsis in peripheral blood is obviously higher than sepsis; Different concentrations of TGF-β1 have different influences of TNF-α,when TGF-β1 is little, there is no effect on TNF-α. When TGF-β1 is high enough, it can stop TNF-α from composing; TGF-β1 can promote IL- 10 composing, thereby promoting inflammation reaction suppression.
