The Application Value Of Sepsis Related Scoring System In Children With Severe Sepsis

Objective: To investigate the feasibility of Sepsis3.0 standard in the diagnosis of severe sepsis in children,and to explore the prognostic value of serum lactic acid,CRP,PCT,FDP,serum bilirubin,serum creatinine,platelet and other indicators,as well as SOFA score,qSOFA score in children with severe sepsis.Methods:A retrospective analysis was performed in the pediatric intensive care unit of the second hospital of Hebei Medical University from January 1,2017 to May 31,2018.According to the Sepsis1.0 standard,51 cases of children with severe sepsis were confirmed,namely,systemic inflammatory response syndrome caused by infection with organ dysfunction.Relevant data of the cut-off points of admission time of the subjects were collected and SOFA score was performed.Sepsis3.0 standard was used to judge the selected children,namely infection and SOFA? 2,and the positive rate and negative rate of this standard for the diagnosis of severe sepsis in children were calculated.According to the prognosis,the selected children were divided into survival group and poor prognosis group,and Logistic regression analysis was conducted on the factors that may affect the prognosis of children with severe sepsis using SPSS18.0 statistical software.Results:A total of 51 children with sepsis were confirmed as severe sepsis according to the Sepsis1.0 standard,of whom 48 were confirmed as sepsis according to the Sepsis3.0 standard,3 were excluded,with a positive rate of94.1% and a negative rate of 5.9%.CRP,FDP,SOFA score were correlated with the prognosis of children with severe sepsis,while serum lactic acid,PCT,platelet,serum bilirubin,serum creatinine,qSOFA score were not significantly correlated with the prognosis of children with severe sepsis.Conclusion:The introduction of SOFA score Sepsis3.0 standard has a certain reference value for the diagnosis of severe sepsis in children,and more clinical data are needed for further improvement.Elevated CRP?FDP?SOFA scores increase the risk of poor prognosis in children with severe sepsis.Serum lactic acid,PCT,platelet,serum bilirubin,serum creatinine and qSOFA scores are not significantly correlated with the prognosis of children with severe sepsis.
