Objective:To observe the clinical characteristics of patients with nonalcoholic fatty liver disease in patients with type2diabetes mellitus, analyze its correlation with microalbuminuria.Methods:Continuous selection253cases of hospitalized patients with type2diabetes, according to the results of liver B ultrasound, were divided into nonalcoholic fatty liver group (NAFLD) and non NAFLD group, and retrospective analysed the clinical characteristics of the two groups and urinary albumin levels.Results:No significant differences in gender, blood pressure, HbAlc levels in the two groups (P>0.05). NAFLD group showed younger, shorter course, higher BMI than non NAFLD group. Microalbuminuria, urinary immunoglobulin G, transferrin, urinary a1microglobulin, urine NAG enzyme, were significant difference between two groups.Non NAFLD group had higher urinary protein loss than in NAFLD group. After using propensity score matching method for balance the two groups at baseline, the above-mentioned differences were still exist (P<0.05). Total cholesterol, LDL-cholesterol, CH-HDL-LDL levels between the two groups were not significant difference (P>0.05). The level of triglyceride in NAFLD group were higher than that of NAFLD Group (P<0.05), but the level of HDL-cholesterol were lower (P<0.05). These disparities continue to exist after the propensity score matching, In non NAFLD group, albumin levels closely related to microalbuminuria levels (P<0.01), but not in the NAFLD group.Conclusion:Nonalcoholic fatty liver disease in patients with type2diabetes mellitus may exist adjustment mechanism in early liver compensatory. The lipid accumulate in liver in order to reduce damage, caused by free fatty acid, to kidney and extrahepatic tissues. Kidney damage is light, so the urinary protein loss is less. This mechanism is independent of the blood pressure, HbA1c level.
