Ulcer Manifestations Of Malignant Lymphoma In Duodenal:Report Of Three Cases And Review Of The Literature

1、ObjectiveBy analyzing three cases collected in our hospital and the10cases reported in the Chinese magazines to raise diagnosis and treatment of lymphoma in abnormal duodenal bulb especially the performance of ulcer2、MethodsThree patients diagnosed as duodenal lymphoma which performance as duodenal bulb culcers in our hospital between2006-2015were analyzed and followed up and10similar cases reported in our country in nearly20years were summarized. However, this study also reviewed the rare diseases which present as duodenal ulcer such as TB. EG、CD、Zollinger Ellison syndrome. Adenocarcinoma.3、ResultAmoning the13patients,8were man (61.5%),5were women(38.5%),the lowest age at diagnosis was25and the oldest age was77,the average age at diagnose was50±27,the middle age was52.76%(10/13) patients complained the upper abdominal discomfort or pain, between them4patient(40%,4/10) complained that Symptoms aggravated with food intaking;3patients complained melena and2patients complained haematemesis.Under pathological diagnosis,7patients were clearly diagnosed DLBCL,3patients as B-NHL,1patients as NHL,1patients as HL,1patient as small B-NHL.As a result,6patients received chemotherapy,6patients surgery,1patient received surgery combined chemotherapy.4、ConclusionDuodenal ulcer sometimes was the manifestation of some rare disease such as TB*EG、CD、Zollinger Ellison syndrome、Adenocarcinoma and Lymphoma. In clinical, we should pay attention to rare disease.Duodenal bulb lymphoma is difficult to diagnose.We should be carefully when patient have refractory ulcer, otherwise biopsy under the endoscopic or ultrasonic endoscope is the main method. Duodenal bulb lymphoma usually performs as DLBCL.The clinical treatment is surgery combined chemotherapy.
