Objective:This study aims to explore the clinical features and the influence of various prognostic factors on survival of patients with multiple myeloma (MM).Methods:A retrospective analysis were performed on clinical data of787multiple myeloma cases in our hospital between January2006and June2014,recording the clinical characteristics, laboratory examination and therapeutic options. Description of the clinical features of patients with MM was studied and13clinical and laboratory parameters were analyzed by univariate and multivariate process.Results:787cases of MM patients cover491males and296females. Median age of the whole group was61years(ranged29-89years old).Since2010, the cases newly diagnosed were increased. The first common symptoms of these patients in the study were bone pain(51.2%)and fatigue (48.0%).Anemia(Hb≤100g/L) was present initially in69.4%patients,and IgG was the most common sub-type(46.6%).52.2%of the patients are diagnosed on stage ⅢIA according to Durie-Salmon,and44.6%are on stage Ⅲ according to International Staging System(ISS). The univaried analysis identified13prognostic factors:age, hemoglobin,Scr,albumin, serum calcium,(32-microglobulin (β2-MG), lactic dehydrogenises(LDH), C-Reactive Protein, bone marrow plasma cells, the differentiation degree of bone marrow plasma cell and therapeutic factor were prognostic factors of overall survival (OS).Multivariate analysis suggested that age, serum calcium, LDH, Scr, the differentiation degree of bone marrow plasma cell and therapeutic factor were all independent prognostic factors for OS.No difference could be found between the Ⅰ group and Ⅱ group of D-S Staging System and between the Ⅱ group and Ⅲ group of International Staging System. Conclusion:The MM patients are relatively middle and old age male patients, clinical characteristics are bone pain and fatigue mainly at diagnosis, the higher rate of the patients newly diagnosed on stage Ⅲ according to Durie-Salmon. Older age, hypercalcaemia, high lactic dehydrogenises, renal insufficiency, the low differentiation degree of bone marrow plasma cell and refusal of treatment were highly related to poor prognosis of MM patients.
