| Objective To discuss the clinical research of contrast-enhanced ultrasound application value in the fallopian tube source sex infertility.Methods1.Choosed 80 cases of tubal source infertility patients were clinical preliminary diagnosisd in in shandong province maternity hospital in January 2013-December 2014.2.Using the American GE company produces the Voluson E8 type multifunction color doppler ultrasonic diagnostic apparatus, equipped with 3 d,4 d transvaginal probe, the frequency:5.0 to 7.5 MHz, USES contrast agents:sound novy (SonoVue) to inspect.3.After angiography repeated playback, dynamically sonographic findings was detailed observation tubal contrast agent through the situation, and a 3 d image reconstruction, further observed oviduct overall situation, the contrast agent dispersion morphology, umbrella end.4.To observe:(1) to observe 80 cases of infertility patients results; (2) comparing ultrasonic oviduct imaging (HyCoSy) and X line oviduct iodine oil radiography (HSG) test results, the results consistency; (3) comparing HyCoSy and laparoscopic tubal commune Dye solution (Lap) and Dye test results, the results consistency; (4) comparing HyCoSy and oviduct fluid test results, the results consistency; (5) comparison of HSG and HyCoSy adverse reactions.5.The data from application us medical statistical software SPSS17.0 statistical, count data using χ2 inspection, the consistency analysis using Kappa coefficient test, P<0.056.howed statistically significant difference.Results1.Contrast-enhanced ultrasound results:2 d HyCoSy:tubal patency article 72; Article 63 the fallopian tube jammed; Fallopian tube obstruction 21, fallopian tube line 126; fallopian tube direction distorted 30; 3 d HyCoSy:tubal patency article 74, article 62 fallopian tube jammed, fallopian tube obstruction of article 20, fallopian tube line nature 114, fallopian tube direction distortion article 42, two kinds of ultrasonic imaging mode is tubal patency of distribution ratio difference, no statistical significance (P> 0.05), two kinds of ultrasonic imaging direction is the distribution patterns are tubal ratio difference, was statistically significant (P<0.05).2. Contrast agent through the fallopian tube time tubal patency group was obviously lower group of patients, fallopian tube jammed is similar between the two groups was statistically significant (t=36.72, P<0.05).3. On the basis of HSG check HyCoSy salpingitis inspection sensitivity is:93.23%, speciality rate:93.24%, the accuracy rate:93.10%, Kappa consistency check (Kappa= 0.665, P<0.05).4.In 58 patients of HSG and laparoscopy checked, including tubal patency article 57, fallopian tube pass unobstructed article 55; HyCoSy check:tubal patency article 55, tubal patency article 57, accuracy is 96.43%; Oviduct through liquid check:tubal patency of article 44, fallopian tube is 68, accuracy was 76.79%, the two methods of inspection is tubal patency of distribution form and accuracy difference was statistically significant (21 =14.16,22=23.06, P<0.05), no statistical significance of Kappa consistency check (Kappa=0.082, P>0.082).5. On the basis of Lap and Dye check HyCoSy salpingitis inspection sensitivity is: 90%, speciality rate:100%, the accuracy rate:93.55%, Kappa consistency check has no statistical significance (Kappa=0.462, P>0.462).6.Bilateral oviduct tubes, unilateral jammed, unilateral obstruction, bilateral jammed unilateral occlusion, bilateral and unilateral jammed congestion level is above moderate incidence of adverse reactions, respectively 0,23.53%,23.81%,23.53%,50%,60%, bilateral oviduct tubes above moderate incidence of adverse reactions was significantly lower than patients with fallopian tube obstruction, fallopian tube jammed comparative differencewasstatisticallysignificant(P<0.05).Conclusion1.2D and 3D real-time HyCoSy in the evaluation of tubal sterility of the fallopian tube patency sex has higher accuracy,3D HyCoSy observed oviduct line advantages more apparent.2.Contrast agent through the fallopian tube time can be used as identification on the basis of tubal patency and jammed.3.The HyCoSy inspection accuracy is higher than the tubal fluid examination.4.HyCoSy and HSQ Lap and Dye test results with high consistency.5.HyCoSy inspection with high security, bilateral oviduct tubes with adverse reactions was significantly lower than patients with fallopian tube jammed patients with tubal obstruction. |
