Objective:1. To investigate the potential mechanism of acute ischemic stroke with gastrointestinal dysfunction diseases from the cell’s point of view;2. To understand the change of electrogastrogram in acute cerebral infarction;3. To evaluate the efficacy of Tongqiao Huoxue Decoction improve the gastrointestinal dysfunction in acute stage of cerebral infarction.4. To explore the potential mechanism of activating blood circulation to dissipate blood stasis in treatment of acute cerebral infarction complicated with gastrointestinal dysfunction.Methods:We used 10 weeks old male Wistar rats as the research object, and established the middle cerebral artery occlusion model(MCAO)by the line embolus.72 rats were screened after Longa test and randomly divided into 4 groups:Tongqiao Huoxue Decoction conventional dose group, Tongqiao Huoxue Decoction high dose group, Domperidone group, and model group. Another 18 rats were established into sham operation group and 18 normal rats as normal control group. The above 6 groups of rats were randomly divided into 24h,4D and 7d three subgroups, a total of 18 groups. Each group had 6 rats, and they were 108 rats in all. Immediately awoke after modeling, Rats in every group were treated by Tongqiaohuoxue Decoction high dose suspension (2.6g/ml), Tongqiaohuoxue Decoction of routine dose ofsuspension (1.3g/ml), domperidone suspension (0.27mg/ml) in proper order respectively, every treatment were 2ml liquid; model group, sham operation group and the normal group was given the same volume of saline.24h subgroups were treated for just 1 time and in gastric electric later guillotined after 24 hours; 4D sub group administered continuously for 4 days after the detection of gastric electrical and death; 7d subgroup administered continuously for 7 days after the detection of gastric electrical and death. The rats were awake after modeling the nervous system defect behavior score; Gastric electrical slow wave amplitude and slow wave frequency are observation indexes; They were killed and immediately cut fresh gastric mucosa, gastric tissue samples were prepared. Light microscopy was used to examine the morphological structure of gastric mucosa tissue.Results:1. The influence of rats with acute cerebral infarction neurological defect behavior:the Longa score results showed that,24h,4D,7d score are lower, compared with the sham operationgroup, there were significant difference (P<0.01).2. To observe the acute cerebral infarction rats’gastric electrical slow wave amplitude and frequency change: Cerebral infarction rats’gastric electrical slow wave amplitude of 24h group and 4D group rats were lower than that in normal group and sham operation group. There is a very significant difference (p<0.01); Cerebral infarction 7d group rats’ gastric electrical slow wave amplitude is lower than normal rats, with statistical significance (P<0.05), while the normal group and sham operation group rats’gastric electrical slow wave amplitudecompared with no difference (p>0.05). Cerebral infarction 4D group than in 24h and 7d groups of gastric electrical slow waveamplitude has the drop tendency. Cerebral infarction of gastric electrical slow wave frequency has no obvious statisticalsignificance.3. Effect of Tongqiao Huoxue Decoction on acute cerebral infarction in rat gastric electrical slow wave amplitude and frequency:Tongqiao Huoxue Decoction high dose group, the conventional dose group and domperidone group gastric electrical slow wave amplitude are higher than cerebral infarction group(p<0.05). Tongqiao Huoxue Decoction high dose group’s gastric electrical slow wave amplitude was significantly higher than that of domperidone group, Tongqiao Huoxue Decoction in conventional dose group (p<0.05), domperidone group, Tongqiao Huoxue Decoction conventional dose group gastric electrical slow wave amplitude showed no difference (p>0.05). Gastric electrical slow wave amplitude from high to low is Tongqiaohuoxue Decoction high dose group> domperidone group-Tongqiaohuoxue Decoction in conventional dose group. The gap was statistically significant. Gastric electrical slow wave frequency has no obvious statistical significance.4. Effect of rat stomach tissue morphology of acute cerebral infarction: Cerebral infarction,24h,4D,7d group rats were visible to the naked eye injury of gastric mucosa. Optical microscopy showed submucosal hyperemia, edema, inflammatory cell infiltration, erosion and ulcer, partially visible petechial hemorrhage. The degree of gastric mucosa damage is the trend of 4D>24h>7d group.5. Effect of Tongqiaohuoxue decoction, domperidone on the morphology of gastric tissue of acute cerebral infarction in rats:Under visual observation, Tongqiaohuoxue Decoction group and domperidone group of rat gastric mucosa had no obvious change. Under the light microscope, Tongqiaohuoxue Decoction high dose group of gastric tissue integrity, a small amount of inflammatory cells, no congestion, haemorrhage and ulcer. Tongqiao Huoxue Decoction conventional dose group gastric tissue of mucosa, submucosal,muscular layer, serosa level clear, mucosal glands with the rules, the infiltration of inflammatory cells reduced obviously, mucosal and submucosal capillary dilatation, hyperemia, edema was significantly relieved, no erosion, ulcer and necrosis. Domperidone group stomach tissue of submucosa and muscularis and serosa level less tidy, mucosal glands with irregular arrangement of mucosal and submucosal blood capillary dilate, still have obvious hyperemia, edema, inflammatory cell infiltration. The degree of gastric mucosal injury was Tongqiaohuoxue Decoction high dose group<domperidone group (p<0.05), Tongqiaohuoxue Decoction high dose group<Tongqiaohuoxue Decoction in conventional dose group (p<0.05), domperidone group and Tongqiaohuoxue Decoction conventional dose group had no statistical significance (p>0.05).Conclusion:1.Acute cerebral infarction rats existed gastric mucosa pathological changes of morphology.2. The strength of contraction of gastric peristalsis inhibited may be one of underlying pathogenesis of the acute cerebral infarction complicated with gastrointestinal dysfunction3. Application of Tongqiao Huoxue Decoction on gastrointestinal dysfunction in acute stage of cerebral infarction has certain protective effect.
