Objective: A retrospective evaluation of percutaneous transforaminal endoscopic discectomy(PTED) and traditional interspinous enlarged short-term clinical Curative effectof two kinds of operation mode of fenestration in the treatment of lumbar disc herniation, to analyze and compare the advantages and disadvantages of PTED and the traditional surgical approach,and to provide clinical reference and theoretical basis for the treatment of lumbar disc herniation.Methods: Counting the statistics of patients with lumbar disc herniation in the Affiliated Hospital of Shandong University of Traditional Chinese spine orthopedic ward hospitalized January since 2014- December 2014, and analyze the criteria and clinical studies included 44 cases met the diagnostic criteria and schedule follow-up of patients with clinical and radiological data.According to the operation were divided into two groups, 22 cases of PTED group, 22 cases of fenestration group. Recording two groups respectively in patients before, during and after the operation of data, including operation time, intraoperative bleeding volume, postoperative intervertebral disc resection volume, absolute bed time, hospitalization time and the JOA score,VAS score, ODI score of preoperation,one month after operation,three months after operation.Analyzing the above results by statistics analysis,and throngh the efficacy of modified Macnab criteria to assess the clinical efficacy of the two groups after three months.Results: All patients were followed up for three months or more and to compare the two kinds of operation curative effect.Clinical efficacy of PTED group of patients, the excellent and good rate was 90.91%, fenestration group was 81.81%, the excellent and good rate comparison in PTED group is better than fenestration group; there was statistical significance to compared the difference of discectomy amount,blood loss, absolute bed rest time after operation between two groups;there was significantly statistical significance to compare the JOA score,VAS score, ODI score of one month after operation,three months after operation with preoperation within group,but there was no statistical significance between group.Conclusion: PTED is a minimally invasive operation with the traditional, it has the advantages of traditional surgical and percutaneous techniques,it can directly reach the disc position to remove the lesion nucleus,to release the oppression to the spinal cord and nerve roots,It can reduce the surgical trauma and to achieve the similar effect and traditional surgery at the same time,it can be used as an effective treatment of pure lumbar disc herniation.
