Objective By comparison analysis of difference between thoracolaparoscopic esophagectomy with transthoracic esophagectomy, to explore its clinical application value.Methods retrospective analysis of 56 cases of second affiliated hospital of Kunming Medical College in 2010-2013 after surgical treatment of esophageal carcinoma patients.According to the different mode of operation, divided into the thoracic Group (31 cases) and endoscopic Group (25 patients),comparison of these clinical indicators different surgical blood loss> regional lymph node dissection> ventilator-assisted breathing time、postoperative tracheotomy、postoperative first day drainage、closed drainage time、lying times、postoperative complications、length of stay,and for statistical analysis. Data were statistically analyzed by using SPSS 17.0, measurement data to average ± standard deviation (x±s). Establishment of an inspection level of a=0.05, to p<0.05 as the difference was significant.Rate of pulmonary complications in endoscopic grou lower than a traditional open group (p<0.05);Results:There were no deaths, and no conversion Thoracotomy. Comparison of clinical indicators:Intraoperative bleeding in endoscopic group was less than the open Group (p<0.05);postoperative first day drainage> closed drainage time、lying times、 length of stay significantly lower than Thoracotomy Group (p<0.05).Average number of regional lymph node dissection between the two groups was not statistically significant (p>0.05); Comparison of postoperative complications:Incidence of postoperative pulmonary complications in endoscopic group lower than the open Group (p<0.05);rate of anastomotic leakage were compared between the two groups was not statistically significant differences (p>0.05);Respirator usage and tracheotomy in endoscopic group was significantly lower than the open Group (p<0.05).Conclusion Thoracolaparoscopic esophagectomy is minimally invasive,radically dissection of lymph node]、low incidence of pulmonary complications and little effect on lung function and operation safety and reliability, shorter hospital stay, and other advantages.It is valuable for clinical application.
