Short Term Outcome Analysis Of Single Compartment Osteoarthritis Treated With Two Kinds Of Total Knee Arthroplasty

Objective:Through the comparison of recent curative effect of unicompartmental knee arthroplasty of knee joint and the total knee replacement in treatment of unicompartmental osteoarthritis of the knee,Study two kinds of operation mode in the treatment of this disease treatment effect,the choice of operation mode and therefore more reasonable,to achieve satisfactory therapeutic effect.Methods : the clinical data of 68 cases of unicompartmental knee osteoarthritis were selected in 2013 April-2014 year in October in the Second Affiliated Hospital of Dalian Medical University Department of joint surgery for the treatment of operation. All the patients age, gender, height, weight and other basic information of no significant difference, comparable. The patients were randomly divided into UKA group and TKA group, 34 cases in each group. Group UKA included 5 males(6 knees), 29 females(32 knees), aged 56-78 years old, average 67 years of age, body mass index 22-32kg/ square meters(average 28kg/ m); TKA group 7 male(7 knees) and 27 female(31 knees), aged 58-79 years old, average 69 years old. Body mass index 23-31kg/ square meters(average 27kg/ m). Two groups of patients by the same surgeon respectively by unicompartmental replacement surgery were treated with total knee replacement surgery, the UKA group and the TKA group of patients with operation time, intraoperative bleeding and postoperative drainage, active knee flexion is greater than or equal to 90 DEG for long, 10 day postoperative pain relief of knee joint function before and after degree and surgery(HSS score) were compared. After surgery to the last follow- up time was 6-18 months, mean 10.6 months. All the patients included in this study were obtained with complete follow-up.Results: all the patients were successfully completed the operation, patients in group UKA and no cases in operation transfer for TKA operation. Group UKA operation when the length of:(74.1 + 20.6) min, TKA group, operation time as:(69.8 + 19.3) min, the comparison between the two had no statistical significance(P>0.05). Compared with the postoperative drainage volume of blood loss in operation: the group UKA intraoperative blood loss and postoperative drainage for(138.2 + 29.5) ml, TKA group of intraoperative blood loss and postoperative drainage for(289.7 + 59.1) ml, blood loss and postoperative drainage volume in the UKA group was significantly less than that in TKA group(P <0.05 value, the difference had statistical significance); active knee flexion is greater than or equal to 90 DEG for long: when long Ping Junwei in group UKA(7.5 + 3.7) d, long Ping Junwei used in the TKA group(9.2 + 4.3) d, used in the UKA group is obviously longer than those in the TKA group, the difference was statistically significance of P<0.05(t=2.3936, P=0.0187); the 10 postoperative day the degree of pain relief in the comparison: UKA group, TKA group, the excellent and good rate reached 94.1%, the excellent and good rate reached 85.2%, suggesting that the two operation methods can effectively relieve pain, two group of comparison, through statistical analysis, the excellent and good rate of UK A group is higher than that of TKA group, there was statistical significant differences(P< 0.05). That in the immediate postoperative period after UKA operation in the treatment of patients with pain relief faster; preoperative UKA group HSS score by(61.2 + 6.4) points increased to at the last follow-up(91.2 + 6.9), TKA group is composed of(62.5 + 6.1) points increased to at the last follow-up(84.3 + 7.1) points. The two group HSS scores were significantly increased(P<0.01). The comparison between the two groups, UKA group of the last follow-up HSS score higher than that of TKA group(P<0.05), the difference was statistically significant.Conclusions: in the treatment of unicompartmental osteoarthritis of the knee, application of unicompartmental knee arthroplasty(UKA) and the knee after total knee arthroplasty(TKA) were able to achieve better therapeutic effect. But comparatively speaking, UKA surgery with s mall trauma, less bleeding, rapid recovery and the maximum retained the normal tissue and body feel and other characteristics, the short-term curative effect is better than the total knee arthroplasty. Worthy of wide popularization and application.
